Description
FEDERAL HIGHWAY ADMINISTRATION STORM DAMAGE REPAIR TO ROADWAYS, CULVERTS, EMBANKEMENTS, BRIDGES, AND OTHER ROADWAY FEATURES, U.S. VIRGIN ISLANDS, ST. CROIX PROJECT NO. VI ST ER STX (003) 693C73-20-R-000007 DESIGN-BUILD (D-B) SOLICITATION PHASE I REQUEST FOR QUALIFICATIONS (RFQ) Project Description:� This project will repair storm damage caused by Hurricanes Irma and Maria in 2017 at 16 sites on 13 different roads on St. Croix, USVI. The proposed improvements include, but are not limited to, removing damaged asphalt concrete pavement, pipe culverts, paved waterways, sidewalks, guardrail, utility lines and poles, bridges, and other debris; excavating for roadways and structures; constructing embankments with or without geogrid reinforcement; placing aggregate base and asphalt concrete pavement; installing pipe culverts, headwalls, drainage inlets, paved waterways, and riprap; cleaning drainage structures and reconditioning shoulders and ditches; constructing bridges, major culverts, and retaining walls; installing guardrail; placing pavement markings and signs; and adjusting, relocating, or replacing utilities.� The work also includes obtaining permits, coordinating utilities, assisting in right-of-way (ROW) acquisition, installing and maintaining erosion and sediment control devices, installing and maintaining temporary traffic control devices, and other miscellaneous work. � The design and construction cost for this base project is estimated to be more than $10,000,000. Design-Build Scope of Work (SOW):� The detailed scope of work to be undertaken by the successful Offeror under the Design-Build contract for this project will be identified in the Request for Proposal (RFP).� However, the work is anticipated to include, but is not limited to, the following: Design and construction of asphalt and concrete roadway pavement, and concrete curbs; Design and construction of drainage inlets, culverts, large box culverts, paved waterways, and headwalls; Design and installation of drainage outfalls; Design and construction of slopes using MSE walls, gabions, or concrete retaining walls; Design and installation of pedestrian sidewalks and railings; Design and construction of bridges; Coordination and acquisition of environmental and construction permits, including the payment of all associated permitting fees; Support for ROW acquisitions for permanent and temporary easements by VIDPW. Design and installation of guardrails; Survey and mapping; Utility designation, location, and mapping, and other utility investigations as necessary; Utility relocations and adjustments, and coordination with local utility owners for the timely completion of utility work; Maintaining utilities throughout construction; Development and implementation of a Transportation Management Plan; Design and implementation of temporary traffic control plans; Design and implementation of temporary erosion and sediment control measures; Quality assurance and quality control for design and construction; and Overall project management, including formal and informal coordination with FHWA-EFLHD, FHWA-PR, VIDPW, and other local government agencies. � The following is a listing of some of the major work elements expected for this project: Design Work: Roadway geometric design. Survey and mapping, deed research, and property boundary location. Utility as-built investigation and verification, including utility location and mapping. Utility test pits for horizontal and vertical location. Coordination and design of utility repairs and relocations. Geotechnical and pavement investigation, analysis, and design for roadway excavation and embankments, cut and fill slope stability, retaining walls, temporary and permanent pavements, major culverts, bridge foundations, etc. Preparation of a Geotechnical and Pavements Report. Hydrologic and hydraulic analysis and design for culverts, inlets, storm sewer systems, ditch capacity and stability, scour protection at bridges and drainage outfalls, etc. Preparation of a Hydraulics Report. Structural design of bridges and major culverts. Roadside safety design, including obtaining and analyzing traffic and crash data for clear zone determination and barrier design. Permanent and temporary pavement marking and signing design. Development of a Transportation Management Plan, including preparation of Traffic Control Plans for all phases of construction.� Identify staging areas and haul routes. Landscaping design for re-establishment of vegetation affected by construction. Soil erosion and sediment control design for all phases of construction, including preparation of a Storm Water Pollution Prevention Plan (SWPPP) for each site. Preparation of plans and specifications, preparation and coordination of submittals for review, and resolution of comments. Preparation of documentation for any proposed design exceptions and/or variances, resolution of review comments, and obtaining final approval of design exceptions and/or variances. Development and implementation of a Quality Management Program for design. Development of right-of-way plans and plats, and design support for the acquisition of any additional right-of-way and permanent or temporary easements necessary for construction.� The DPW will acquire ROW.� The Contractor is responsible for preparing right-of-way plans, and preparing and recording cadastral survey plans.� Prepare right-of-way plans with sufficient detail to allow the appraiser to discuss the property with the owner.� The DPW will obtain titles, prepare and review appraisals, and complete the acquisition of the properties.� Any additional warranty deed or drainage easements required due to design changes will be the responsibility of the Contractor to develop in coordination with DPW ROW staff.� A representative from the DPW must be present for any discussions with property owners, unless waived in writing prior to the discussion. Preparation of permit applications, preparation and coordination of submittals for review, resolution of comments, and obtaining all required permits for construction of the project. Community and Stakeholder involvement and coordination, including documentation of meetings. Design documentation (record drawings and as-built plans). Preparation of shop drawings during construction. Construction Work: Construction survey stakeout, and placement of right-of-way monuments. Clearing and grubbing. Demolition and removal of structures and obstructions within the construction limits. Grading. Construction of retaining walls. Preparation of subgrades and construction of the approved pavement designs. Construction of drainage systems including culverts, storm drains, inlets, headwalls, curb and gutter, paved waterways, scour protection measures, and other drainage structures. Construction of bridges and major culverts. Adjustment, relocation, or replacement of utility conduit, cable, pipes, poles, manholes, and other appurtenances that are damaged or conflict with the proposed construction.� This includes coordinating the design and construction with the affected utility companies. Construction of guardrail and permanent barriers. Installation of permanent pavement markings and signing. Installation of permanent erosion and sediment control measures, including turf establishment and installation of landscaping. Installation and maintenance of temporary erosion and sediment control measures, including management of soil stockpiles within the project limits. Maintenance of public traffic through work zones.� This includes construction of temporary pavements, and placement of temporary pavement markings, construction signs, barriers, and other traffic control devices. Maintenance of utilities during construction, including protecting, adjusting, or relocating utilities, in coordination with local utility companies. Obtaining all permits for construction, and implementing permit requirements. Compliance with the environmental commitments as identified in the Categorical Exclusion. Coordination with federal and state agencies for environmental inspection and compliance including, but not limited to, the Environmental Protection Agency and VI Department of Planning and Natural Resources, Division of Environmental Protection. Development and implementation of a Public Relations Program to inform the public of upcoming construction activities and address public concerns. Coordination with adjacent construction projects, including private development work and other DPW projects. Coordination with affected property owners, EFLHD, DPW, and other agencies as applicable. Development and implementation of a Quality Management Plan for construction.� The Quality Management Plan includes, but is not limited, to providing independent construction inspection services, performing material testing, and providing product warranties. Project administration, management, and scheduling. Preparation of construction plan revision submittals. Preparation of as-built plans, including completion of an as-built survey of the finished work. � Procurement Overview of the Project:� In accordance with Public Law 104-106 (41 U.S.C. 253m) and FAR 36.301 titled Use of two-phase design-build selection procedures, the EFLHD is conducting a ""two-phased"" D-B selection process.� Under this Notice, the EFLHD is soliciting D-B firms/teams to participate in Phase One � Qualifications Based Selection � of this ""two-phased"" D-B selection process. � A selection committee will review and evaluate the Statement of Qualifications (SOQ) submitted by all responsive D-B firms/teams (Offerors).� The selection committee will select up to three of the most highly qualified Offerors to participate in Phase Two of this D-B selection process based on the established RFQ evaluation criteria.� During Phase Two, those Offerors short-listed will be sent a Request for Proposal (RFP), which will outline the remainder of the selection process requirements.� The RFP will contain the requirements necessary for the short-listed Offerors to submit both a Technical Proposal and a Price Proposal during Phase Two.� Those Offerors will be required to provide the necessary information to the EFLHD within 30 calendar days after receiving the RFP.� A site visit/pre-proposal meeting will be held within the first 14 calendar days (at the project location in St. Croix, USVI) of this Phase Two period to provide additional information.� It is anticipated that a stipend will be awarded to each Phase Two Offeror who provides a responsive, but unsuccessful proposal as per 23 CFR 636.112 CFR 636.113.� A copy of the draft RFP may be available to interested Offerors during Phase One. � After receipt of the Phase Two Technical and Price Proposals, the Contracting Officer will conduct a ""best value"" analysis as per FAR 36.303-2 titled Phase Two. �Award of one Firm Fixed Price contract is anticipated by approximately July 2020, with Notice to Proceed issued approximately August 2020.� Construction is to be completed by June 30, 2022. � Evaluation and Weighting Criteria:� All Offerors responding during Phase One will be evaluated using the following criteria and weightings: � Offeror Past Performance and Experience (Maximum Score = 30). �The Government will review and evaluate the information about each Offerors' past performance and experience in the design and construction of roadway and bridge projects based on the Offeror's record and reputation for satisfactorily completing projects similar in size, scope, and complexity.� Please submit information in tabbed format as organized below:� Section 1A:� Submit documentation that shows direct experience of the Offeror, joint venture partners, Offerors related by an ownership agreement, and/or experience of subcontractors that the Offeror proposes to utilize in the execution of this D-B project.� Identify any projects where the lead contractor and the lead designer have worked together, on the same project, in a Design-Build relationship. Section 1B:� The lead design firm and lead construction contractor shall each list at least three (3), but no more than five (5), relevant roadway and bridge projects, including any D-B projects, performed in the past ten (10) years.� The lead construction contractor's projects shall be those with a contract value of greater than $3,000,000 that were completed in the last ten (10) years, and that were similar in scope to the proposed design and construction work for this project.� Offerors must identify any projects where the lead contractor and the lead designer have worked together, on the same project, in a Design-Build relationship.� If any D-B project is provided as a reference that was not performed together by the lead design firm and lead construction contractor, then the Offeror shall identify the design professional or construction entity with whom it contracted for each such project. Section 1C:� Submit documentation that shows evidence of performance with past and present customers; past and present subcontractors; Federal, State, and local government agencies; utilities owners; and/or consumer protection organizations.� Include names, titles, email addresses, and phone numbers for all references submitted.� Highlight examples of design and/or construction work involving EFLHD and/or the VI. Section 1D:� Submit three past performance evaluations from those owner agencies on projects of similar scope within the past ten (10) years, indicating that quality work was completed on-schedule and within budget.� Include any statements as to the Offeror's integrity, reasonable and cooperative conduct, and commitment to QA, QC, and customer satisfaction.� Submit a minimum of two (2) references. �Include agency or firm�s name, point of contact name, email addresses and phone number, date work was performed and nature of work. Key Personnel and Organizational Structure (Maximum Score = 30). �The Government will review and evaluate each Offeror's design and construction experience, including technical and geographical similarities between the Offeror's design experience and the work described in this solicitation.� Provide experience of key personnel working together as a team, and a description of measures used to partner or form team cohesion between construction, design, and subcontractors. Submit documentation for the key design and construction personnel listed below who will be involved in the design and construction of the project, indicating their specific experience within the past ten (10) years performing work that is similar in scope to the proposed design and construction activities for this project.� Indicate the percentage of time the key personnel will dedicate to the project.� Personnel should not hold multiple key roles, as identified below, in the Team structure. �As a minimum, include documentation for the following key personnel, each having the required experience performing technically similar work in the Virgin Islands, Caribbean, and/or nearest states: Design-Build Project Manager - The Offeror�s principal point of contact and primary coordinator with all applicable agencies for all activities throughout the entire design-build process.� This individual shall have a minimum of two (2) years of experience within the past five (5) years performing technically similar work, including at least one project with a contract value greater than $3,000,000. Designer of Record - The Offeror�s single point of responsibility for all design procedures and decisions. �This individual shall be a licensed Professional Engineer licensed in the local jurisdiction, and have a minimum of two (2) years of experience within the past five (5) years performing technically similar work, including at least one project with a contract value greater than $3,000,000. Quality Assurance Manager (QAM) - The Offeror�s designee responsible for providing Quality Assurance and Quality Control of the work, and ensuring conformance with the Contract documents.� The QAM is responsible for 100% review and oversight of all QC activities.� This individual shall be a licensed Professional Engineer having a minimum of ten (10) years of supervisory experience in roadway and bridge design or ten (10) years of supervisory experience in inspection or materials testing on highway and bridge transportation construction projects, or a combination thereof. Quality Control Manager (QCM) � The Offeror�s designee responsible for providing Quality Control of the construction work, and ensuring conformance with the Contract documents.� The QCM is responsible for the oversight of all construction activities.� This individual shall be a licensed Professional Engineer having a minimum of ten (10) years of supervisory experience in roadway and bridge design or ten (10) years of supervisory experience in inspection or materials testing on highway and bridge transportation construction projects, or a combination thereof. Permitting and Utility Coordinator � The Offeror�s single point of contact for coordinating with permitting agencies and utilities.� The Offeror may separate the permitting and utility coordinator responsibilities by assigning a permit coordinator separate from the utilities coordinator.� This individual shall have a minimum of two (2) years of experience within the past five (5) years performing technically similar work.� � Construction Superintendent � The Offeror�s authorized representative in charge of the construction work.� This individual shall have a minimum of ten (10) years of experience as a construction superintendent performing technically similar work, including at least two (2) projects within the past five (5) years, and one project with a contract value greater than $3,000,000. � Bridge Engineer - The Offeror�s authorized representative in charge of the bridge and box culvert design. This individual shall be a licensed Professional Engineer with at least ten (10) years of experience performing technically similar work on bridge and box culvert projects, including at least (2) projects within the past five (5) years. Hydraulic Engineer � The Offeror�s authorized representative in charge of the drainage analysis and design, including, bridges, culverts, open channels, scour countermeasures, storm water management features, and erosion and sediment controls.� This individual shall be a licensed Professional Engineer with at least ten (10) years of experience performing technically similar work. � Geotechnical and Pavement Engineer � The Offeror�s authorized representative in charge of the geotechnical and pavement analysis and design, including the design of foundations, retaining walls, slope stability measures, and rigid or flexible pavements. �This individual shall be a licensed Professional Engineer with at least ten (10) years of experience performing technically similar work. Submit the organizational chart for each Offeror, identifying major participants and describing their roles in the project.� Submit a communication plan identifying how each partner and subcontractor will interact and fit into the overall D-B team.� Identify any anticipated Disadvantaged Business Enterprise (DBE) subcontractors. Obligation to keep team intact: In connection with the services covered by this procurement process, any in-house personnel, subcontractors, and outside associates or consultants will be limited to the individuals or firms that are specifically identified during the RFQ submittal process. The contractor shall obtain the Contracting Officer�s written consent before making any substitution for these designated in-house personnel, subcontractors, associates, or consultants. Key personnel cannot hold more than one position. Quality Management Plan (Maximum Score =10). �The quality management plan consists of the quality control plan (QCP) and quality assurance plan (QAP) for design and construction activities. Prepare a Quality Management Plan that establishes a clear distinction between quality control (QC) and quality assurance (QA) activities, and specifies the personnel performing each function.� The Government will review and evaluate each Offerors' quality management plan to assess the soundness and reasonableness of the Offeror�s planned QA and QC for Design and Construction of all materials on the project.� The plan should be specific to this project. Submit a description of the system to be used to manage the design and construction schedule, and the management system to control cost and quality of materials.� Describe how the systems will be updated and monitored.� Explain how the Offeror will provide QA and QC for both the design and construction elements of the project, including the names of QC managers, the names of independent testing laboratories, and the number of personnel at various certification levels planned for the Project. Describe how the QA and QC organization will operate, including how it will interface with the Offeror's organization, EFLHD, and USVI.� The Offeror should demonstrate how QA and QC functions will be performed independently to maintain the integrity of the QA and QC process. Submit evidence of construction performance within the past seven (7) years, indicating that quality work was completed on-schedule and within budget.� Include any statements as to the Offeror's integrity, reasonable and cooperative conduct, and commitment to QA/QC and customer satisfaction.� Submit a minimum of two (2) references. Financial Condition (Total Maximum Score = 10).� The Government will review and evaluate each Offerors' financial condition to perform the design and construction of the project. Submit the most recent audit report performed by the Defense Contract Audit Agency, or similar audit, reflecting the Offeror's rates and detailing the makeup of cost pools from which the rates are derived.� Include audits for all joint venture partners and architect-engineering firm subcontractors. �Include a Dun and Bradstreet Comprehensive Report, or a similar report, prepared by an independent business reporting agency that provides a company profile to include the number of employees; a debt to asset ratio; payment history; a credit evaluation/rating and a comparison of this rating to other firms in the same industry; and a history of the Offeror's ownership. Describe any project that the lead contractor or lead designer were involved in within the past seven (7) years that resulted in: (a) the assessment of liquidated damages against one of such parties; (b) claims being submitted by or against one of such parties that involve the project owner; (c) one such parties having received a notice to cure a default due to the party�s non-performance or poor performance of the underlying contract; or (d) one of such parties being terminated for cause. For any such situation, explain the circumstances and identify the project owner�s representative and his/her current telephone number. Disclose any outstanding litigation that could materially and adversely affect the financial condition of the lead contractor and/or the lead designer. Project Understanding and Approach (Maximum Score = 20).� The Government will review and evaluate each Offerors' understanding of tasks involved and approach to managing, designing, and constructing the project. Submit a general description of the tasks involved in the project and the approach the Offeror will take for design and construction administration, including the involvement of the design members of the Offeror's team during construction.� Identify potential risk factors, special issues or problems that are likely to be encountered and explain the approach to mitigate those risks, issues or problems.� Identify a general description of the planned public involvement and coordination.� Include discussion on how the Offeror intends to maximize the benefits of the D-B process. If the Offeror is a limited liability company, joint venture or any form of partnership, provide complete copies of the organizational documents that allow, or would allow by the time of contract award, the Offeror to do business.� In addition, the Offeror shall provide a letter from a surety or insurance company (with a Best�s Financial Strength Rating of A minus and Financial Size Category VIII or better by A.M. Best Co, or approved equal) stating that the Offeror is capable of obtaining a performance and payment bond based on the current estimated contract Describe the means and methods by which Offeror will plan and control the scheduling of work to meet the contractual completion date(s), including any plans which address the possibility of early completion.� Phase One Submittals:� Interested Offerors may use the Standard Form (SF) 330 U.S. Government Architect-Engineer Qualifications, as the basis for their SOQ.� SF 330s are available on the U.S. General Services Administration website at:� http://www.gsa.gov/forms/.� Although SF 330s are formatted for Architect-Engineer and Related Services, the construction elements of the Offeror's SOQ may also be included on the SF 330.� Alternate forms and formats may be submitted, provided that the applicable information from the SF 330 is included.� Each Offeror must provide a clearly marked original and eight (8) copies of the completed SOQ, most recent financial audit report, and an electronic version via CD/DVD.� The SOQ should contain no more than forty (40) pages typed on one side only.� The Phase One SOQ shall be bound and tabbed with the major headings.� If tabs contain project-specific information they will be counted as pages.� Provide one audit report which should be packaged separately and will not count against the forty (40) page total.� The SOQ shall be written on 8.5"" x 11"" paper.� All SOQs shall be written using Times New Roman with a font of 12-point.� The SOQ should address each of the rating categories in the same order as listed above. It is not the intent of EFLHD to receive project specific design or engineering recommendations as part of this Phase One selection process.� Offerors should limit their SOQs to the information required by this Notice, and other information that may be relevant to the qualifications and experience of the Offeror.� No reimbursement will be made for any expenses associated with the preparation of Phase One SOQs.� Send one (1) original, eight (8) copies, and electronic version of the completed SOQ to the following address: ����������� Federal Highway Administration ����������� Eastern Federal Lands Highway Division ����������� 21400 Ridgetop Circle ����������� Sterling, VA� 20166 ����������� Attn: �Melvin Sloan ����������� (Fax and email submissions will not be accepted.)
