- This amendment contains the following changes: 1) The following equipment has been removed from Section D.2.1.1.4 NON-CONSUMABLE/CONSUMABLE MEDICAL SUPPLIES: ? Removed ""Sharps Container"" from BASIC LIFE SUPPORT (BLS) LINE PROVIDER EQUIPMENT LIST (EMTF) ? Removed ""Fire Extinguisher"" from all levels of care equipment lists as fire extinguishers are required and addressed through Section D.2.1.1.5 CONTRACTOR PROVIDED VEHICLE ? Removed ""Transcutaneous Cardiac Pacemaker"" from ADVANCED LIFE SUPPORT(ALS) LINE PROVIDER EQUIPMENT LIST (EMPF) 2) The following Language has been added to Section D.2.1.1.4 NON-CONSUMABLE/CONSUMABLE MEDICAL SUPPLIES: In regards to the BLS and ILS Line Provider Equipment List AED requirement and the ALS Line Provider Equipment List Monitor/Defibrillator-Manual requirement, a statement has been added to clarify the inventory requirements of those specific pieces of equipment. The statement is as follows: **Equipment is not required for each line provider to have with them in their possession on the line, but each EMS agency must have the equipment at the incident and available per IDAPA 16.01.03 500.01 which states, ""Each EMS agency must maintain sufficient quantities of medical care supplies and devices specified in the minimum equipment standards to ensure availability for each response."". The following is replaced to Section D.16 NORTHERN ROCKIES PRE-AWARD INSPECTION d) Copy of individual EMTFs, AEMFs and/ or EMPFs verified fireline qualifications (See Section D.2.1.1.3 TRAINING/EXPERIENCE. Note: For EMTFs, AEMFs and/or EMPFs who were verified by a Northern Rockies Equipment Inspector/Personnel Verifier MOU holder in 2018 or 2019 may submit the signed Individual Employee Training Verification form certifying fire qualifications in lieu of full fireline qualification records as listed below. *****Offerors unable to submit fireline qualification certifications by the April 17 deadline, but are otherwise qualified for an award, may be awarded and suspended until such time as D.16.1 PERSONNEL FIRELINE QUALIFICATION RECORDS VERIFICATION is met. ***** e) List of consumables with restocking fees. Submittal must be in the form of a word document. Should an award take place, the list will be attached to the agreement by modification for claim purpose. See D.2.1.1.4 NON CONSUMABLE/CONSUMABLE MEDICAL SUPPLIES. SUBMITTALS DUE April 17, 2020 at 1200 MST (see exception in D.16.1 below). Submittal cover shall include company name, contact information, and the number of pages attached. Submittals may be mailed or delivered to Contacting Officer at the address listed in Block 9 or scanned and emailed to debby.wesselius@usda.gov. Faxed copies will not be accepted as the resolution is not acceptable.
