- SOLICITATION The Central Arkansas Veterans Healthcare System (CAVHS) has a requirement for regulated medical waste (RMW) collection transportation, and disposal at the following locations: Main Campus at 4300 W. 7th Street, Little Rock, Arkansas North Campus at 2200 Fort Roots Drive, North Little Rock, Arkansas Conway CBOC at 1520 East Dave Ward Drive, Conway, Arkansas Searcy CBOC at 1120 South Main Street, Searcy, Arkansas Russellville CBOC at 3106 West 2nd CT. Russellville, Arkansas Hot Springs CBOC at 177 Sawtooth Oak Street Hot Springs, Arkansas Pine Bluff CBOC at 2906 Market St. Pine Bluff, Arkansas and, El Dorado CBOC at 514 E. Sty Street, EL Dorado, Arkansas The contractor shall provide all labor, tools, materials, equipment, transportation and supervision necessary to complete all work associated with providing regulated medical waste (RMW) and reusable sharps container management & disposal services in complete and strict accordance with the resultant contract and all attachments thereto. The expected ordering period is intended to be 7/1/2020 through 6/30/2025 but may be adjusted based on the actual date of award. This is a 100% Service-Disabled, Veteran-Owned, Small Business (SDVOSB) set-aside under NAICS code 562211, with a small business size standard of $41,500,000.00. All responsible SDVOSB s may submit a quotation, which shall be considered by the agency. All offerors must be registered as an SDVOSB in the U.S. Department of Veterans Affairs, Vendor Information Pages (VIP) (https://www.vip.vetbiz.va.gov/) as of the date you re your quotes is received and the date of award, as well being registered in the System for Award Management (SAM) at http://www.sam.gov as of the date of award. In accordance with solicitation provision 52.237-1, an organized site visit will be conducted on Wednesday, March 25, 2020, at 1:00PM local time. See the cover page of the solicitation for specific information regarding this site visit. Interested parties must submit all questions concerning the solicitation, in writing, via email to the Contracting Officer. Questions must be submitted by email to james.hunt2@va.gov and received no later than March 27, 2020. Questions submitted after this date may not be answered. Quotes are required to be received in the contracting office no later than 11:59PM local time on April 9, 2020, or the date indicated in block 8 of the Standard Form 1449. As indicated on the cover page, quotes must include the following to be considered for award: - Contractor s business name (as registered in SAM) - Contractor s address (as registered in SAM) - Contractor s DUNS number (as registered in SAM) - Contractor s CAGE code (as registered in SAM) - Point of Contact (POC) Name - POC phone number - POC email address - Completed Price Schedule (Solicitation Section B.1) - Completed Offeror Representations and Certifications (Solicitation Section E.7) - Completed Technical Acceptability Questionnaire (Solicitation Exhibit A) - Customer name, POC, email, phone number, contract number and contract amount for each reference submitted in response to Solicitation Exhibit A, Question 9.
