Description
The Department of Veterans Affairs, James A. Haley Veterans Hospital and its outlying clinics (New Port Richey, Brooksville, Lakeland, South Hillsborough and Zephyrhills, Florida (hereafter referred to as James A. Haley Veterans Hospital) is currently seeking information from any Service-Disabled Veteran-owned Small Business (SDVOSB) or Veteran-owned Small Business (VOSB) vendor, or other small businesses (SB) that can provide the following. 1. Scope of Work: Employee Assistance Program Services-Provide provide programs and services by an independent, local counseling service for assessment, referral and limited counseling. (SEE ATTACHED STATEMENT OF WORK) 2. Performance Period:� Base Year: April 1, 2020 to March 31, 2021 Option Year 1: April 1, 2021 to March 31, 2022 Option Year 2: April 1, 2022 to March 31, 2023 Option Year 3: April 1, 2023 to March 31, 2024 Option Year 4: April 1, 2024 to March 31, 2025 � 3. Place of Performance: James A. Haley Veterans Hospital and its outlying clinics to include: New Port Richey, Brooksville, Lakeland, South Hillsborough and Zephyrhills, Florida. Interested Vendors The applicable NAICS for this service is 624190. This industry comprises establishments primarily engaged in providing nonresidential individual and family social assistance services (except those specifically directed toward children, the elderly, persons diagnosed with intellectual and developmental disabilities, or persons with disabilities). To be considered for this requirement, vendors must be registered in the System for Award Management (SAM -https://beta.sam.gov/).
