Amendment 0001: This amendment is to revise the prices on Attachment A to reflect the cost of two meetings per period on CLIN0002, CLIN0004,CLIN0006, CLIN0008, CLIN0010.� Acknowledgement of the amendment and utilization of the updated Attachment�A - Schedule of Prices document is required for responses�to this solication to be considered complete.� __________________________________________________________________________________________ (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This is a request for Quotation (RFQ) is being issued via solicitation number 75H70420Q00008. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This requirement is Indian-owned Small Businesses Economic Enterprise (ISBEE) and the associated NAICS is 561920, Convention and Trade Show Organizers a with small business size standard of $11 million. (v) The Contract Line Items (CLINs) and pricing structure can be found in Attachment A �Price Structure� attachment. (vi) The Indian Health Service (IHS) requires event management support services that will allow the Agency to sponsor an effective work group meeting for the Purchased/Referred Care (PRC) Program. The work group will discuss improvements for PRC and provide IHS direction on improving this important program for American Indian and Alaska Native people.� Meeting attendees will total approximately 50 people. The IHS requires more than an event�s physical occurrence � they require an atmosphere that is conducive to facilitating collaborative thinking and disseminating information among attendees. All offerors submitting proposals must complete the Indian Firm Representation & Tax Liability forms accompanied with this solicitation.
