- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C25720Q0377 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. SET-ASIDE: This requirement is a 100% Total Service-Disabled Veteran Owned Small Business (SDVOSB) set aside under the authority of 38 U.S.C. 8127(d). In order to be eligible for award an offeror must be certified/verified in Vetbiz (www.vip.vetbiz.gov), and small for the applicable NAICS code at the time of the quote submission and at the time of award. Status as a qualified SDVOSB concern is under the authority of 38 CFR Part 74 in accordance with the VA Acquisition Regulation (VAAR) Part 819. Eligibility determination is performed by the Center for Veterans Enterprise (CVE). Only qualified offerors may submit bids. Combined Synopsis/Solicitation: 36C25720Q0377 replaces Combined Synopsis/Solicitation: 36C25720Q0143. 36C25720Q0143 is cancelled and no quotes will be accepted regarding it. An amendment will follow at a later date cancelling Combined Synopsis/Solicitation 36C25720Q0143. NAICS Code: 492110 and Size Standard is 1,500 (In Number of Employees) Introduction: The Government intends to award a firm fixed price base with four option year renewal contract based on this solicitation. Description of Services: The Department of Veterans Affairs, VA North Texas Health Care System (VANTHCS) Pharmacy Service located at the Dallas Veterans Medical Center, Dallas, Texas has a need for small package pharmacy delivery services. The Contractor shall provide regularly scheduled pickups of prescriptions from various locations as noted below and deliver to pharmacy locations throughout the VA North Texas Healthcare System or to patient homes. (End of Clause) 852.219-74 LIMITATIONS ON SUBCONTRACTING MONITORING AND COMPLIANCE (JUL 2018) LIMITATIONS ON SUBCONTRACTING MONITORING AND COMPLIANCE (a) This solicitation includes VA Acquisition Regulation (VAAR) 852.219-10, VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside. (b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement. Washington, DC 20420 REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE SYSTEM (CPARS): As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all contracts that exceed $100,000 and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, the Contractor Performance System (CPARS), which is maintained by the National Institutes of Health (NIH). The report shall be assigned in CPS to the contractor s designated representative for comment. The contractor representative will have thirty days to submit any comments and re-assign the report to the VA contracting officer. The Duncan Hunter National Defense Authorization Act of 2009 (Public Law 110-417) was enacted on October 14, 2008. Section 872 of this Act required the development and maintenance of an information system that contains specific information on the integrity and performance of covered Federal agency contractors and grantees. The Federal Awardees Performance and Integrity Information System (FAPIIS) were developed to address these requirements. FAPIIS is a distinct application that is accessed through the Past Performance Information System (PPIRS) and is available to federal acquisition professionals for their use in award and responsibility determinations. FAPIIS provides users access to integrity and performance information from the FAPIIS reporting module in the Contractor Performance Assessment Reporting System (CPARS), proceedings information from the Central Contractor Registration (CCR) database, and suspension/disbarment information from the Excluded Parties List system (EPLS). The Government intends to award one contract resulting from this solicitation. Please read each section below carefully for the submittals and information required as part of the evaluation. Failure to provide the requested information below shall be considered non-compliant and your quote could be removed from the evaluation process. Offeror quotes shall be evaluated under FAR Part 13.106-2(b) -- Evaluation of Quotations or Offers. Therefore, the Government is not obligated to determine a competitive range, conduct discussions with all contractors, solicit final revised quotes, and use other techniques associated with FAR Part 15. The contracting techniques associated with FAR Part 15 are not mandatory. The Government shall award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation. The Contractor shall provide regularly scheduled pickups of prescriptions from various locations as noted below and deliver to pharmacy locations throughout the VA North Texas Healthcare System or to patient homes. Polk Street, After Business Hour(s): From Dallas VA Medical Center To: Intermittent Locations (Patient Address to Be Determined) Other locations consist primarily of other hospitals, patient residences, and vendor sites; but may also include locations not mentioned in this SOW as required by Dallas VAMC Pharmacy. Period of Performance: Base plus four (4) one-year option renewals. Base Year: April 1, 2020 March 31, 2021 Option Year 1: April 1, 2021 March 31, 2022 Option Year 2: April 1, 2022 March 31, 2023 Option Year 3: April 1, 2023 March 31, 2024 Option Year 4: April 1, 2024 March 31, 2025 Cargo: Items to be transported will consist primarily of medications but shipments may also include miscellaneous supplies (i.e. bandages, needles, iv fluids) All shipments will be clearly labeled. The Contractor will not be responsible for packaging and labeling medications and/or supplies. Contractor Responsibilities: The contractor assumes all liability risks for work performed under this contract. Must assume total liability for all contract employees. CONTRACT EMPLOYEES ARE NOT EMPLOYEES OF VANTHCS; Must provide 24 hour, 7 days per week stand-by service. All couriers transporting medications under this contract shall possess a valid driver s license, vehicle insurance, and registration and have two-way radios, cell phones, or pagers for communication purposes. The Contractor shall provide proof that all drivers have had training in the transportation of medication. Any delays in deliveries shall be reported to Stephanie Carey at (214) 857-0556. The Contractor shall provide all vehicles and couriers needed to perform this effort. Contractor shall ensure the confidentiality of all patient information and records being transported and will be held liable in the event of a breach of confidentiality. The Contractor shall comply with the provisions of the Federal Privacy Act of 1974 (Public Law 93-579). The C&A requirements do not apply and a Security Accreditation Package is not required. The Service Contract Act applies to this requirement and the appropriate SCA Wage Determination is attached. All applicable employees performing under this contract shall be paid in accordance with the SCA. The Contractor shall maintain proper temperature in vehicles to preserve medical supplies, and medications. The Contractor shall provide insulated transport containers for the pickup/delivery of medications. Vehicles shall not be left in extreme heat or cold while medications are being transported. Occasionally, the Contractor shall be required to furnish dry ice for transportation prior to arrival at the pickup site. Dry ice is most often used to transport certain medications/supplies and is not expected on a routine basis. Pickup/delivery schedule: Pick up and deliveries shall be made daily from each clinic (Monday-Friday) except on Federal Holidays (as observed). Pickup times will be established for each clinic by Dallas VAMC Pharmacy Service in accordance with clinic personnel. The Contractor shall give prior notification to each clinic at the phone numbers indicated below if the scheduled pick-up time cannot be met for any reason. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday and any other day specifically declared by the President of the United States to be a national holiday. The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government. The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this performance work statement. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel. Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length in excess of three inches, while assigned under this contract. CONTRACTORS ATTIRE AND APPEARANCE: Contractor personnel shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities. The Government reserves the right to review Contractor personnel qualifications. Upon request, resumes shall be provided to the Contracting Officer prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority. CONTRACTING OFFICER REPRESENTATIVES: Prior to contract award, the Contracting Officer shall designate a VA Medical Center employee as the COR. All work coordination shall be made through the COR. The Contractor shall be provided a copy of the letter of delegation authorizing the COR at the commencement of the term of the contract. No other person shall be authorized to act in such capacity unless appointed in writing by the Contracting Officer.
