- This notice is for Solicitation Number 36C24220B0028 - 561-14-106 Upgrade Bathroom & Drinking Fountains at the VA NJ Healthcare System (East Orange Campus), 385 Tremont Ave, Building 6, East Orange, NJ 07018 This shall be a Service Disabled Veteran Owned Small Business (SDVOSB) Set-Aside. As a result, all SDVOSB's may submit a Proposal, which will be considered by the agency. This project is classified under NAICS Code 562910 and has a Size Standard of $22.0 million. The cost estimate range for this project is $25,000 and $100,000. STATEMENT OF WORK (SOW) Project # 561-20-114 Remove Bldg. #6 underground storage tank 1. Contract description. Removal of 1000 gallon gasoline underground storage tank. 2. Background. Existing underground gasoline storage tank has approx. 600 gallons of fuel which is unusable (stale). There is no means of removing that fuel because the pump is malfunctioning and beyond repair. The tank and pump need to be removed as per DEP and EPA. 3. Scope. Removal of obsolete non-serviceable 1000-gallon gasoline underground storage tank and all contents (approx. 600 gls. of unusable gasoline), with LSRP services, testing of surrounding soil for contamination, apply for any and all local, county, and state permits and restoration of disturbed area with clean fill and resurfacing with asphalt. 4. Specific Tasks. The scope of work shall include but not be limited to the following main items: Use oversite form LSRP Licensed Site Remediation Professional and follow their guidelines and procedures for duration of project. LSRP will file notice of intent and retention for site. Inform NJDEP 14 days prior to tank closure field activity. Provide field supervision of closure and sampling for tank field area taking approx. 15-20 samples with lab analytical costing. Provide summary and closure report. Follow NJDEP, state, county and local laws. Apply for all necessary permits. Demolition and removal of Concrete fueling island, dispenser, bollards and tank cover pads. Demolition of electrical circuits for dispenser. Excavate to expose one 1000-gallon underground storage tank. (Tank #06-1 registration UST180004). Remove all petroleum waste and disposal by a licensed facility. Cut open and clean tank. Remove tank from ground. If groundwater is found it will be sampled for contamination and remediated. Test and remediate any contaminated soil. Backfill excavation with certified clean backfill material. Transport tank to scrapyard for disposal. Restore area disturbed with asphalt to match existing. Provide receipts for record. A Response Action Outcome (RAO) is to be generated by the LSRP as final remediation closure document. Immediately after receiving the contract award, the Contractor shall; Provide all necessary Labor, Material and equipment to perform the required work to restore work area to usable space. 5. Performance Monitoring (if applicable). Work shall be coordinated with facility thru the Contracting Office Representative (COR). COR will monitor the contractor work performance on daily basis and the contractor is required to submit daily s progress report to the COR. 6. Security Requirements. General Contractor s employees shall not enter the project site without appropriate security badge. They may also be subject to inspection of their personal effects when entering or leaving the project site. No photography of VA premises is allowed without written permission of the Contracting Officer except as specifically required by the scope of the work. VA reserves the right to close or shut down the project site and order General Contractor s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. Not Used b. Identification of Potential Conflicts of Interest (COI). Not Used e. Inspection and Acceptance Criteria. The COR is responsible for certifying that the work done under the contract is performed to time and standard. They are also responsible to assure the inspection and acceptance of products provided incidental to services. 9. Risk Control Contractor shall take all measures for safety & infection control and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered. Contractor shall permit access to Department of Veterans Affairs personnel and patients through other construction areas which serve as routes of access to such affected areas and equipment. Coordinate alteration work in areas occupied by Department of Veterans Affairs so that Medical Center operations will continue during the construction period. 10. Place of Performance. Parking lot area next to the Building #6 at NJHCS, East Orange NJ Minimize interference of construction activities with flow of Medical Center traffic, keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction materials, debris and standing construction equipment and vehicles. 11. Period of Performance. Contractor to start immediately after receiving the NTP, completion time is 45 calendar days.
