Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 36C24720Q0220 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 562119, Other Waste Collection. This is set-aside for Small Businesses. The requirement is for use at the Carl Vinson VA Medical Center. Delivery location is Carl Vinson Veterans Health Administration Medical Center, 1826 Veterans Blvd, Dublin, Georgia 31021. ****NOTE: Please reference information in the section of the Statement of Work Vendor/Contractor Requirements The Vender/Contractor must be locally owned and operated and have landfill privileges in Laurens County, and hold waste disposal licenses and/or permits in the State of Georgia to ensure waste disposal is done in accordance with all local and state regulations and keep fuel costs and emissions to a minimum. The Contractor shall provide qualified and trained employees and transportation vehicles that meet the Department of Transportation requirements for waste and recycling transport. The Vender shall not sub-contract any part of this service to a third-party contractor under any circumstances. The Contractor must be able to physically respond, on site, to emergencies within one hour of notification. 10.0 Description Please note #4 Clean inside and outside area of the VAMC owned 40 cubic yard Sani Pac compactor once per month
