- SOURCES SOUGHT NOTICE: Firm Fixed Price Contract (FFP) DESCRIPTION: This is a sources- sought to determine the availability of potential sources having the skills and capabilities necessary to provide Fabrication and installation of Wall, panels, Sinks and Counter Top Services, at Department of Veterans Affairs, located at the he William Jennings Bryan Dorn VA Medical Center (WJBDVAMC) in Columbia, SC. The period of performance is February 24, 2020 through March 27, 2020. Please be aware all Federal, State and local labor laws will be applicable to this procurement including, federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act and the McNamara-O Hara Service Contract Act. If you have specific questions regarding labor laws, you should contact your local Federal Department of Labor representative. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources- sought synopsis. Questions should be submitted by email to wileen.stokes@va.gov Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman-owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 332999 All Other Miscellaneous Fabricated Metal Product Manufacturing (size standard 750 Employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to the sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to the sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to the sources sought announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, two (2) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced later, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. STATEMENT OF WORK Installation of Wall Panels, Sinks, and Counter tops 1. GENERAL: This requirement is for a firm fixed price contract for fabricate and installation of wall panels, sinks, and counter tops at Department of Veterans Affairs, William Jennings Bryan (WJB) Dorn VA Medical Center located in Columbia, South Carolina. 2. BACKGROUND: The WJB Dorn VA Medical Center is a 216-bed facility, encompassing acute medical, surgical, psychiatric, and long-term care. The hospital is located in Columbia, South Carolina and provides primary, secondary, and some tertiary care. 3. PERIOD OF PERFORMANCE: From Award date up to 30 days. Monday Friday, 8:00 a.m. to 4:30 p.m. excluding federal holidays. No overtime is authorized unless approved by the delegated Contracting Officer Representative (COR) and/or Alternate Contracting Officer Representative (ACOR) for required testing. 4. PLACE OF PERFORMANCE: WJB Dorn VA Medical Center 6439 Garners Ferry Road Columbia, SC 29209 5. PERFORMANCE REQUIREMENTS: A Contracting Officer Representative (COR) and/or Alternate Contracting Officer Representative (ACOR) shall be assigned to this procurement and written notification will be provided to the Contractor after award. Scheduling of work performance shall be arranged through the delegated COR/ACOR assigned. Any work performed after working hours shall be approved and scheduled by the COR as required. The Contractor shall furnish all labor, transportation, equipment, materials, tools, supplies, supervision, incidental engineering, shipping, and management required to perform the fabricate and installation of wall panels, sinks, and counter tops Scope of Work Required Contractor fabricate and installation of Wall Panels, Sinks, and Counter tops necessary to ensure that complies with applicable local, standards, accordance with ADA requirements for wheelchairs and manufacturers standards of safety, reliability, and satisfactory conditions while performing work at the VA Medical Center. The awarded contractor shall perform a site inspection to validate the dimensions listed below. It is the contractor s responsibility to accurately measure each piece of material prior to fabrication and follow-on installation. The contractor shall provide 15 days advance notice prior to the delivery of the material to allow for hospital notifications and prework. 1. Fabrication and Installation Color: Mojave Material: CORAIN solid surface panels C Installation of three (3)-Vanity Under Mount Charge- three sinks under mount Color Glazier White Work Excluded Repair or replacement made necessary due to negligence or misuse of the equipment by persons other than the Contractor, his representatives or his employees. Connections to existing plumbing fixtures. The COR or designee must approve in advance any extra charge work attributed to vandalism or misuse. No invoice will be paid without prior approval. Any repairs due to misuse or negligence shall be billed according to the labor rate specified in the pricing schedule. Parts will be billed at actual invoiced. Warranted Equipment Fabrication, Installation, and Parts other than that installed under this contract shall not be removed, replaced, or deficiencies corrected while still under warranty of the manufacturer of the installer without prior approval of the COR. All defects in material and workmanship, defective parts, or improper installation and adjustments found by the Contractor shall be reported to the COR in writing prior to leaving the facility. Check-In Requirements Contractor an all Contractor s Service Personnel shall check in and out with Engineering Control Center (ECC), Building 100 prior to performing and upon completion of all required service. Licensing and Supervision The Contractor shall have 5 years experience on Fabrication and Installation of Walls, countertops, and sinks Evidence of all qualifications of personnel shall be provided to the COR, The Contractor shall manage and provide proper Supervision, the total work effort associated with the operations, repair and all other services required herein to assure fully adequate and timely completion of these services. The Contractor shall provide an adequate staff of personnel with the necessary management expertise to assure the performance of the work is in accordance with sound and efficient management practices. The Contractor shall implement all necessary work control procedures to ensure timely accomplishment of work requirements, as well as to permit tracking of work in progress. The Contractor shall plan and schedule work to assure material, labor, and equipment are available to complete work requirements within the specified time limits and in conformance with the quality standards established herein. Verbal scheduling and status reports shall be provided when requested by the COR and followed-up with written documentation Staffing The Contractor shall continuously maintain an adequate staff with suitable management expertise to assure work is scheduled and completed in accordance with these specifications. The Contractor shall maintain an adequate craft work force to complete work in accordance with the time and quality standards specified. Work Schedule The Contractor shall schedule and arrange work so as to cause the least interference with the normal occurrence of Government business and mission. In those cases where some interference may be essentially unavoidable, the Contractor shall make every effort to minimize the impact of the interference, inconvenience, equipment downtime, interrupted service, customer discomfort, etc. See each location s special requirements for locations which require special scheduling of work. AVAILABILITY OF UTILITIES The Government shall furnish the following utility services at existing outlets for use in those facilities provided by the Government and as may be required for the work to be performed under the contract: Electricity Natural Gas Fresh Water Sewage Service Refuse Collection (from existing collection points) Refuse collection excludes used/broken wall, Sink, and countertop. Parts and supplies which shall be disposed of by the Contractor off the VA facility premises. Information concerning the location of existing outlets may be obtained from the COR. The Contractor shall provide and maintain, at his/her expense, the necessary service lines from existing Government outlets to the site of work. SAFETY The following practices shall be observed, at a minimum, during maintenance, inspection, or testing procedures: All applicable OSHA standards shall be followed for all work. The Contractor shall provide a copy of their company s safety manual/policy to the COR prior to beginning work. All safety devices must be in operational condition. Lockout/Tag out procedures must be followed if maintenance procedures require that the equipment not be operated. Ensure that personnel performing Fabrication and installation tasks wear clothing that is not loose fitting and that they are provided with proper protective equipment, such as safety shoes, hard hats, eye protection, and hand protection. Provide proper lighting. Ensure the working area is clean and dry. Contractor shall follow all VA regulations and procedures Contractor shall comply with all project infection control risk assessment and must be signed off prior to start of construction activities. Contractor must install all containment barriers that are required by infection control as well as maintaining a clean construction area at the end of each workday. Contractor shall ensure all tools and equipment are secured at all times. Tools and equipment must be removed from open areas or stored in secured location during breaks, lunch, and/or at the end of each work day. All debris must be removed from the facility upon completion of service each day. Contractor shall ensure proper signage, barriers, and/or blockage of service area is provided where applicable, especially at hoist way doors and clearly visible to ensure patient/employee safety is adhered to. Smoking is not permitted within the facilities at any time. All smoking will be conducted in delegated smoking areas only. Unserviceable parts and components shall be removed from Government property unless directed otherwise by the COR.
