- STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, parts, supervision and other items and services necessary to perform software service and maintenance to support the Thermo Fisher Scientific ION Reporter Server System, Ion Chef SVC, and the Ion S5 plus sequencer at the Washington DC Veterans Affairs Medical Center, 50 Irving Street NW, Washington, DC 20422. All work completed by the Contractor shall be performed by a certified Thermo-Fisher technician and/or engineer. The contractor shall provide parts, labor and travel for remedial repair. Planned maintenance visits are included in the service agreement for the two devices. The planned maintenance is to occur once per year for each device. Technical support can be reached by priority telephone and email access. Also, remote monitoring and diagnostic service is available and allows for notification of instrument failures or errors. 1. EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. 2. AREAS TO BE SERVICED: Washington DC VAMC 50 Irving Street NW Washington, DC 20422 2. PERIOD OF PERFORMANCE: 2/1/2020 1/31/2021, with 4 subsequent option years. 3. TASK FREQUENCY AND INSTRUCTIONS: The contractor has a guaranteed priority response time of two business days after a service call is received in Zone 1 or Zone 2. If the contractor fails to arrive within two business days, the company will provide the customer a service plan renewal credit in an amount equivalent to one day s pro-rated charge for each day the contractors response is late. The contractor has a targeted response time of three days for remedial repairs outside of Zone 1 or Zone 2. A service call must be received by the contractor s service center by 2:00 pm local time for priority response time service. Planned maintenance visits are budgeted for an annual visit for each of the two medical devices being supported by the service agreement. The contractor is able to perform more than one annual planned maintenance visit during the contract period, at their discretion, at no additional cost. If the customer requires an additional planned maintenance visit, the customer will be charged for the visit. Contractor shall submit all removable media to be used on a VA system to Biomedical Engineering for scanning with anti-virus software prior to use on the system. In the case of equipment turn-in, exchange, repair or replacement hard drives used by the VA shall be removed from the equipment and remain in possession of VA this includes loaned or rented equipment.
