Description
Justification and Approval for: Other than Full and Open Competition The Brockton and Jamaica Plain VA Medical Healthcare Systems intends to negotiate on a sole source basis pursuant to FAR 6.302 1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements with Creative Dental located in Dedham, MA 02026, or its authorized dealers for the purchase of Dental Lab Items with one day turnaround. 1 Dental Laboratory Services: Those laboratory procedures that are required for the treatment of dental patients. These would include , but not be limited to, all laboratory fabrication aspects of complete dentures, removable partial dentures, fixed partial dentures and crowns, and dental implant prosthesis. PERFORMANCE WORK STATEMENT (PWS) DEFINITIONS VA Boston Healthcare System Jamaica Plain and Brockton Campuses Special Handling: Unusual circumstance may dictate the need for a dental order to be picked up, specially shipped and returned same day, overnight, or processed within a shorter time frame. Routine Handling: A dental laboratory work authorization that is usually performed and returned within a specified time frame. Turn Around Time (TAT): The length of elapsed time between pickup of the dental lab order by the contractor until the receipt of the complete order by VA Dental Service. COR: Contracting Officer s Representative. SAD: Standards of Acceptable Deliverables. Dental Laboratory Services: Those laboratory procedures that are required for the treatment of dental patients. These would include, but not be limited to, all laboratory fabrication aspects of complete dentures, removable partial dentures, fixed partial dentures and crowns, and dental implant prosthesis. Working Day: Any twenty-four hour period of the typical administrative work week, i.e. Monday through Friday (M-F). For example, if the contractor is allowed 5 working days for a prosthesis and is picked up on a Monday, it would be due back to VA the following Monday. Federal holidays occurring M-F are not considered a working day. TERM OF CONTRACT The proposed contract period is for one year 1 with four (4) 12-month option periods, one FAR 52.217-8 six month extension if exercised Anticipated Period of Performance: Base Year 03/01/2020-02/29/2020 Option Year 1 03/01/2021-02/29/2021 Option Year 2 03/01/2022-02/29/2022 Option Year 3 03/01/2023-02/29/2023 Option Year 4 03/01/2024-02/29/2024 Six Month Option 03/01/2024-08/31/2024 SCOPE The Contractor shall provide all resources necessary to accomplish the requirements described in the Performance Work Statement (PWS). The Contractor shall provide dental laboratory services outlined in this PWS. The contractor shall furnish local pickup Monday through Friday between the hours of 8:00 a.m. and 4:00 p.m. Eastern standard time. The contractor will have overnight capabilities to provide urgent orders. All labor, transportation, materials, general dental laboratory tools, equipment and supervision required to provide Dental Prostheses to the VA Boston Healthcare System, Jamaica Plain Campus 150 So. Huntington Ave. Jamaica Plain, MA 02130, Brockton Campus 940 Belmont St. Brockton, MA 02301. The Contractor requires bills to be sent on a weekly basis. No changes are to be made to the Rx without the permission of the sending provider. Contractor must have products that are 100% American Made. The dental laboratory services that may be authorized under this contract include, but are not limited to: Fabrication of porcelain fused to metal crowns and fixed partial dentures Fabrication of Full gold crowns and fixed partial dentures Implant supported crowns and fixed partial dentures Abutments Resin-bonded bridges Removable partial denture frameworks Diagnostic wax-ups for crown and bridge (C&B) cases Custom provisional C&B prostheses All-ceramic C&B prostheses Implant connecting bars (precious metal/titanium) C&B metal/titanium/zirconium substructures Application of veneering porcelain to existing/VAMC submitted metal or ceramic C&B copings and fixed partial denture (FPD) substructures. Removable Prosthesis Full Denture Set-up Full Denture Finish Denture relines and repairs Partial repairs Nightguards Bite blocks Base trays The Contractor shall provide sufficient management to ensure that the work specified in this contract is performed efficiently, accurately, on time, and in compliance with the terms and conditions of the contract. The Government reserves the right to add additional dental products and/or services, within the scope of the original contract, via a Bi-lateral modification. GENERAL REQUIREMENTS The Contractor shall provide services and support to VAMC Boston as defined herein. The Contractor shall fabricate dental prosthetic items in strict accordance with the dental laboratory order for the fabrication of dental prostheses, which shall include but not be limited to the following; all laboratory fabrication aspects of complete dentures, removable partial dentures, fixed partial dentures and crowns, and dental implant prosthesis. Special handling may be required on specific orders and will be authorized on a case by case basis by the VAMC. Special/customized fabrication standards may be required on specific orders and will be authorized on a case by case basis by the VAMC. The contractor will be a full service dental laboratory that could integrate the combined treatment needs of the prosthetic patient requiring simultaneous fixed, removable and implant prosthetics. The normal turnaround time (in working days as defined) for dental prosthetic items shall be within 1 day ARO but not more than 2 days ARO. In case of longer turn-around times, the Contractor must notify the COR of the delay, in writing, as soon as the delay becomes known and before the maximum number of days has expired. This notification must include the reason for delay and the requested amount of additional time. This Notice of Intent is NOT a request for competitive proposals. Parties may identify their interest, accompanied by supporting literature, to the Contract Specialist no later than 2/6/2020 at 11:59 p.m. Responses should be sent to Mary.Bosch@va.gov. Information received will to determine whether or not to conduct a competitive procurement. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer.
