- Sources Sought Information Request Form � Project Name: Service Member and Family Support of the Oregon National Guard State Volunteer Workshop. � Brief Description: The Service Member and Family Support (SMFS) of the Oregon National Guard requests a vendor to provide conference space, catered meals and lodging as outlined below in order to conduct an State Volunteer Workshop on event April 17 to 19, 2020 as outlined below: Location:� Eugene, Oregon, or within a 10 mile radius Conference Space: Workshop: April 17th, 2020 � State Volunteer Workshop � One (1) Registration areas near entrances that allow 75 adult guests ability to register for the event as they arrive. One (1) conference room, to comfortably accommodate 60 people in banquet rounds style seating and 32 people in 16 six (6) foot tables. Extra room available for activities and additional tables/chairs as requested. Audio/Visual requirements for this conference room to include: one (1) podium, one (1) projector, and (1) projector screen, two (2) wireless microphones, and sound patch.� A/V package to be included during all conference hours. One (1) conference room, to comfortably accommodate 100 people in banquet rounds style seating. Used for Awards Banquet � Recognition program with slide show and awards ceremony. Can eat banquet Dinner in this room. Extra chairs and open space needed for additional activities and A/V requirements. 3 additional (6ft. tables on front wall) and extra room available for activities and additional tables/chairs as requested. Audio/Visual requirements for this conference room to include: one (1) podium, one (1) projector, and (1) projector screen, two (2) wireless microphones, and sound patch.� A/V package to be included during all conference hours. Workshop: April 18th, 2020 � State Volunteer Workshop One (1) conference room, to comfortably accommodate 60 people in banquet rounds style seating and 32 people in 16 six (6) foot tables. Extra room available for activities and additional tables/chairs as requested. Workshop: April 19th, 2020 � State Volunteer Workshop � One (1) conference room, to comfortably accommodate 60 people in banquet rounds style seating and 32 people in 16 six (6) foot tables. Audio/Visual requirements for this conference room to include: one (1) podium, one (1) projector, and (1) projector screen, two (2) wireless microphones, and sound patch.� A/V package to be included during all conference hours. Two (2) 8 ft. rectangular tables near the podium along the wall. One (1) 8 ft. rectangular table outside of the conference room for check-out. The (2) conference rooms must meet the following criteria: Located in close proximity to each other and provide limited access to only our registered guests for safety purposes. Power should be accessible for small appliances provided by our personnel such as laptops, projector, and additional technology. Located near a bathroom that can be cordoned off from other users of the facility to be used only for our event. No hotel rooms will be used to meet this requirement. All conference rooms must be located on the same property. All conference rooms must be indoors. �All conference rooms must stay the same for the entire event. Once a conference room is provided during the event, it will continue to be able to be used by the government until the entire event has concluded. Cost will be inclusive of all taxes and service charges. All setup and breakdown of tables and other vendor provided equipment will be conducted by the vendor. Wi-Fi readily available throughout the facility from 7:00 am � 11:00 pm. Meals: April 17, 2020:� Snack for 75 attendees and Dinner for 100 attendees April 18, 2020:� Breakfast for 75 attendees, Lunch for 75 attendees and Snack for 75 attendees.
