Description
In Home Sleep Study Services for the Aleda E. Lutz, VAMC (Saginaw, MI), the VA Ann Arbor Healthcare System (Ann Arbor, MI.) and all Community Based Outpatient Centers VAMC This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date. This Sources Sought/Market Survey Notice is being conducted by Network 10 Contracting, PO Box 492, Ann Arbor, MI 48106 to identify other sources that are capable of providing Sleep Monitoring services that include equipment, monitoring, and training to eligible beneficiaries of the Department of Veterans Affairs, which includes the Ann Arbor, VAMC in Ann Arbor, Michigan, Aleda E. Lutz, VAMC in Saginaw, Michigan and all of its Community Based Outpatient Centers (CBOCs). The Contractor shall supply the monitoring equipment to be used in the patient s home with all supplies needed to complete the test. The Contractor shall contact each patient and provide assistance (over the telephone or in person) required for each night s set-up. The contractor shall provide a Board-Certified Sleep Medicine Physician (BCSMP) who will review and interpret the raw data of the sleep study epoch and provide a report to the ordering physician at the appropriate VA Medical Center. Period of Performance: 12 months from date of Award plus five, 12-month option periods. Hours of Service: 24 hour a day, 7 days a week hotline service and access to a live US based Clinician.
