- Page 5 of 5Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing RALS ACCUCHEK INFORM II. This is for the Wilkes Barre VA Medical Center in Wilkes Barre, PA 18711. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Verify that the business is under 511210 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point-of-Contact, if in possession of a Federal Supply Schedule contract, and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Eugene.Moran@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 P.M. (EST) on Friday, November 22, 2019. No submissions will be accepted after this date and time. Questions can be submitted electronically to Eugene.Moran@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey.RALS Statement of Work I. SCOPE OF WORK: The Remote Automated Laboratory Systems (RALS) Software technology System License Agreement is part of the point of care testing connectivity software that is used to post patient test results to the electronic patient record (VISTA/CPRS). The RALS software enables review and release of patient results to the LIS directly from each unit. It allows the ability to manage, update, review and troubleshoot quality control results, operator and device issues remotely. This software allows the end user to�monitor tests done at the point of care and to meet numerous accreditation requirements for laboratory tests performed at the Butler VAMC.This module is specific for the Accu-Chek Inform II. There are 16 analyzers currently connected to Butler VAMC VistA using RALS.������ The software license agreement ensures a continuity of services for this medical center utilizing the server which is already in place. The term of this license agreement shall commence as of the license effective date and shall remain in effect as long as the STS is licensed hereunder. The term of each license granted hereunder shall remain in effect for duration of the purchase order. The Technology License includes service, support, updates and hardware maintenance. The supply of RALS-Plus for the use by Customer is governed by the terms and conditions of the RALS Software Technology System License Agreement.Period of Performance: One base, plus 4 option yearsBase Year 12/31/2019 12/30/2020Option Year One (1) 12/31/2020 12/30/2021Option Year Two (2) 12/31/2021 12/30/2022Option Year Three (3) 12/31/2022 12/30/2023Option Year Four (4) 12/31/2023 12/30/2024The contractor shall provide RALS Web 3 devices modules and interfacing and download sites to include:1. Software Components:RALS-Web 3 device software module for the Accuchek Inform IIRALS Remote Connect (RRC), if applicableOther third party software, depending on system specifications.2. Other Components:RALS User Documentation will be provided in the current medium.Standard Training, instruction and consultation for the Customer required for the use of the RALS STS.RALS Download Site connectivity3. Hardware Components RALS Dataports if applicable Information Management System4. Standard training, instruction, and consultation for the customer required for the use of the RALS modules5. RALS Download site connectivity hardware components RALS dataports if applicableThe term of this license agreement shall commence as of the license effective date and shall remain in effect as long as the STS is licensed hereunder. The term of each license granted hereunder shall remain in effect for duration of the purchase order. The VA only commits to purchase orders of up to a year (12 months). II. CONFORMANCE STANDARDS: The contractor s performance must be in compliance with all applicable state, federal and local laws and regulations.III. HOURS OF WORK: The Warranty is in effect for the entire duration of the contract. The contractor provides 24 hour, 7 days a week Technical Support by calling 1-877-627-7257. The Technical Support is available Monday through Friday, 8:30 a.m. to 5:30 p.m., EST. After business hours including weekends and holidays, an answering service is available with a 30 minute call back. IV. PREVENTATIVE MAINTENANCE: 1. The Contractor shall provide free upgrades at no cost to the medical center 2. The Contractor provides documentation of services performed. EMERGENCY REPAIR SERVICE:24/7 availability of technical support. VI. PARTS The contractor shall furnish all parts as described in the Scope of Work section as covered by this contract, in accordance with the Conformance Standards Section (section II). End of Document
