SOURCES SOUGHT NOTICEForNephrology and Dialysis Services for Northern Indiana Health Care System, Fort Wayne INThis is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. This Sources Sought/Market Survey Notice is being conducted by Network 10 Contracting, 8888 Keystone Crossing Blvd Ste 1100, Indianapolis, IN 46240 to identify other sources capable of providing Onsite Nephrology Physician and full Dialysis Services, to include equipment, technicians, and trained nursing as applicable, to eligible beneficiaries of the Department of Veterans Affairs Northern Indiana Health Care Systems, located at 2121 Lake Avenue, Fort Wayne, Indiana 46805. The Contractor shall provide 1.0 FTE Board Certified /Board Eligible Onsite Nephrologist Physician, 1.0 Patient Care Technician, 1.0 Nurse Manager, 1.0 Charge Nurse (RN/LPN), 1.0 Medication Nurse, 1.0 Biomedical Technician; and shall maintain certification throughout the period of performance. The contractor shall provide all dialysis equipment to operate a 3-4 bed unit in both outpatient and inpatient care settings.Period of Performance: 12 months from date of Award plus four, 12-month option periods. Hours of Service: 8:00am to 4:30pm Monday- Friday, with potential of Saturday. Contract physician and staff shall work efficiently to complete all tasks prior to the end of the normal eight-hour workday. However, the contract physician and staff are expected to remain on duty until the last patient has been seen; some on-call may be required for emergent patient needs within the Emergency Department or ICU.Responses shall include: (1) Business Name, (2) Point of Contact Name, Phone Number, and E-mail Address (3) DUNS and NAICS code (4) Business Size SMALL or LARGE (5) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUB Zone, WOMEN-OWNED, etc. and (6) PAST PERFORMANCE INFORMATION for Same or Similar Services.Eligible sources are encouraged to reply to this sources sought, as this information may be used to determine potential set asides for the above noted service.
