This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued.a. Project Title: Generator Repairb. Description: VA Long Beach Healthcare System is seeking a contractor to diagnose and repair a Cummins Onan 600KW generator.c. Project Location: VA Long Beach Healthcare System, 5901 East 7th Street, Long Beach, CA 90822d. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26220Q0139.e. Set Aside: This solicitation is 100% set aside for Service Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127f. Applicable NAICS code: 811310, Commercial and Industrial Machinery and Equipment (except Automotive and Electronic) Repair and Maintenanceg. Small Business Size Standard: $7.5 Millionh. Type of Contract: Firm Fixed Pricei. Period of Performance: 120 days from awardj. Wage Determination: Wage Determination No.: 2015-5614, revision 142. Statement of Work2.1 Scope. Contractor shall troubleshoot, diagnose and repair Cummins Onan 600KW, 60HZ, 277V/480V, 900-AMP Generator in Bldg 126.2.2. Specific Tasks2.2.a. Service technician will be required to check in with facility technician before preforming any repairs or services.2.2.b. Contractor shall teardown, troubleshoot and diagnose Generator 126OP. 2.2.c. Contractor shall provide all parts and labor to repair generator.2.2.d. Contractor will always be required to wear all Personal Protective Equipment (PPE). 2.2.e. Any damage caused by the Contracting Personnel will be repaired or replaced by the Contractor with no costs to the VA.2.2.f. Contractor shall remove all debris, provide dust control and keep entire area clean and orderly each day.2.2.g. Contractor shall install a secondary emergency power shut off switch/alarm and light on the exterior west wall of generator 126OP building.2.2.h. Contractor shall provide all parts and labor required to install power shut off switch. 2.2.i. Contractor shall perform startup of all equipment to include a 4-Hour Load Bank (ONE (1) HOUR AT 30% LOAD, ONE (1) HOUR AT 50% LOAD, and TWO (2) HOURS AT 80%) LOAD.2.2.j. Contractor shall fence-off all working areas that are in Pedestrian Pathways, set-up Safety Barriers in all other work areas with Delineators and Caution Tape to ensure the Safety of Staff and Patients. 2.2.k. Contractor and Employees to attend a 30-minute VA Safety briefing provided by the Safety Department.2.2.l. Upon completion contractor will contact POC (Gustavo Flores) to validate completion of required services.2.2.m. Contractor will be required to provide a company letter head digital report to the POC of all repair findings and services performed to generator 126OP.2.2.n. Contractor shall be responsible for disposing of all unused material and cleaning work area prior to leaving facility.2.3. General Requirements2.3.a. Contractor shall provide all Labor, Parts, Materials, Equipment, and Supervision required to perform all work.2.3.b. Contractor shall be responsible for repairing damages incurred during performance of this scope of work to include removal/installation or moving materials and tools in and out of the work area.2.3.c. Contractor shall provide all State, City, County, Federal, OHPOD, and AQMD permits as required.2.3.d. Contractor shall provide a turn key system upon handing over to the VA.2.3.e. Contractor shall follow all policies (Parking, Safety, Smoking, Security, etc).2.3.f. SMOKING POLICY. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas.2.4. Safety Requirements2.4.a. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.1372.4.b. Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely.2.5. Security Requirements. 2.5.a. All Vendor personnel are required to wear I.D. badges during the entire time he/she is on Government property. I.D. badges must have an identification picture and shall state the name of the individual and the company he/she represents. Contractor and Employees shall report to the POC, Gustavo Flores, to be issued Temporary ID Badges before proceeding to the job site. Vendor shall also check-out with the COR and Police Service before leaving the job site.2.5.b. It is the responsibility of the Vendor s personnel to park in the appropriate designated parking areas. Parking information is available from VASDHS Police Service. VASDHS will not validate or make reimbursement for parking violations of the Vendor s personnel under any circumstance.2.5.c. Vendor will not have access to patient records, data or VA computer systems2.6. Duty Hours.
