- 36C25020Q0123SPECIAL NOTICENOTICE OF INTENT TO SOLE SOURCEBiom 5 Retinal Lens SystemThe Cleveland Department of Veteran Affairs Medical Center, 10701 East Blvd Cleveland, OH 44106 intends to solicit and negotiate with only one source under the authority of FAR 13.1061(b)(1)-Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available. The NAICS code is 333314 with a small business size standard of 500. The VA intends to solicit and negotiate with Oculus Surgical, Inc 562 NW Mercantile Place Port St. Lucie, FL 34986. The eye clinic at the Cleveland Louis Stokes VAMC is requesting the procurement of Biom 5 retinal lenses. The Biom 5 is a retinal operating viewing system that works with the Stereoscopic Diagonal Inverter (SDI). The BIOM 5 mounts onto the existing SDI and allows for wide-angle observation of the fundus during vitreous surgery. Currently the LSCVAMC has the Leica SDI, Biom 3 and Biom 4 along with supporting attachments. The Biom 5 shall be compatible and interchangeable with the existing equipment made by the OEM. This procurement is needed to supplement the current supply of Biom 5 retinal lenses due to an increase in surgical cases in the eye clinic.Other persons may identify their interest and capability to respond to this requirement by Wednesday November 20, 2019 at 4PM EST. Such capabilities information will be used solely for the purpose of determining whether to conduct this procurement on a competitive basis. �All information shall be furnished at no cost or obligation to the Government.� A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
