DeBakey VA Medical Center 2002 Holcombe BLVD; Bldg. 100 Houston, TX 77030DescriptionThis is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued.This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019-06 (effective 10/10/2019), Commercial Checklist; VAAR Update 2008-19 (effective 10/04/19). Solicitations for the acquisition of commercial items prepared using FAR Part 12 shall be assembled, to the maximum extent practicable, using the format at FAR 12.303.The associated North American Industrial Classification System (NAICS) code for this procurement is 541380, with a small business size standard of $16.5 million. Registration will be verified in System Award Management (SAM). Quotes will be considered non-responsive and rejected if the Contracting Officer is not able to verify and view contractor registrations in SAM.The Michael E. DeBakey VA Medical Center has a requirement for the Semi-Annual Inspection and Certification of Pharmacy Biological Safety Cabinets and clean rooms to include particle counts per the most current USP 797and 800 requirements. Pharmacy sampling requirements require the following: Semi-Annual certification of six (3) cleanroom testing buffer rooms, three (3) cleanroom testing ante rooms, fifteen (15) biological safety cabinets (BSC s), four (4) horizontal laminar airflow hoods (LAFH s), five (5) horizontal LAFH s, twelve (12) bacterial sampling w/I.D., twelve (12), fungal air sampling per location, twelve (12) bacterial and fungal surface sampling, twelve (12) bacterial/fungal air sampling three (3) IV robots, six (6) glove box/isolators, and three (3) smoke visualization studies completed by a CETA certified technician. Alarm calibration shall be required for all equipment. Period of Performance (PoP) dates: December 18, 2019 thru December 17, 2020 (PoP is subject to change if necessary); with four (4) option years, exercisable at the Government s discretion. Questions are due November 22, 2019 by 11:00am (CT). An amendment will be posted to the solicitation afterward with all questions and answers. If there are no questions, an amendment will not be posted. All interested companies shall provide quotation(s) for and complete items identified in solicitation attachment: S02- Atch_CS Sections B-E_36C25619Q1435 ; complete and submit: Section B: Contract Administration Data and Section E: 52.212-2 Evaluation-Commercial Items, and Instruction to Contractor; award shall be made in accordance with Lowest Price Technically Acceptable (LPTA). Quotes may be considered non-responsive and rejected if the Contracting Officer is not able to verify evaluation factors requested and clearly identified in Section E: 52.212-2 Evaluation-Commercial Items and Instruction to Contractor for a successful submission of quote.
