Description
Department of Veterans Affairs, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide annual service for preventive maintenance, repair and software upgrade for two (2) Medivators Advantage Plus Endoscope Reprocessor systems. SERVICES REQUIRED: Contractor shall provide a performance plan to accomplish preventive maintenance, repair and software upgrades. The PWS shall detail the amount of labor, required tasks, materials, parts covered and customer technical support twenty-four (24) hours a day. The services are for the Department of Veterans Affairs, New Jersey Healthcare System, East Orange Campus, 385 Tremont Avenue, East Orange, NJ 07018. Period of Performance will be one (1) year from date of award with provision for four (4) Option Years. Contractor shall provide services for two (2) Medivators Advantage Plus Endoscope Reprocessor systems. The NAICS code for these services is 811219 (Other Electronic and Precision Equipment Repair and Maintenance) INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to do provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. Responses must be submitted by 3:00 PM EST November 14, 2019. Responses to the information requested above may be submitted via e-mail to Monique.Paltan@va.gov This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. PERFORMANCE WORK STATEMENT (PWS) Background. The Department of Veterans Affairs Medical Center, New Jersey Healthcare System, East Orange Campus has two Medivators Advantage Plus Endoscope Reprocessor systems. In order to keep these systems in optimal functioning order, we require annual service for preventive maintenance, repair and software upgrade. Scope. Contractor shall provide a performance plan to accomplish preventive maintenance, repair and software upgrades. The PWS shall detail the amount of labor, required tasks, materials and parts covered. The scope of this PWS shall be for the two (2) Medivators Advantage Plus Endoscope Reprocessor systems located at GI Lab, Bld.1, 2nd Floor, New Jersey VA Medical Center. It is also required that the contractor provide customer technical support twenty-four (24) hours a day seven (7) days a week. All work should be in accordance with the Original Equipment Manufacturers (OEM) standards. Applicable Documents. Contractor must adhere to guidance and instructions provided in the OEM repair and maintenance manuals. Contractor must comply with all applicable Federal, State, and Local regulations and policies including and not limited to OSHA, VA Safety and Security Policies, etc. Contractor Quality Assurance. Contractor shall provide a checklist with worksheet originals indicating work to be performed and actual values obtained. Contractor must maintain all operational and technical documentation such as operational and service manuals, schematics, diagnostic software and parts list. Contractor must maintain an Inspection Record Log. For each service visit a detailed Equipment Service Report will be required. Each service report shall include: Name of the contractor and contracting number. Name of the FSE performing services. Contractor Service ESR number/log number. Date, time (starting and ending), equipment downtime and hours on-site for service call. Identification of equipment to be serviced; ID number, manufacturer s name, device name, model number, serial number and any other manufacturer s identification numbers. Description of problem reported by Biomedical Engineering Section. Signature from the FSE performing services described. Signature from an authorized VA employee who witnessed services described Contractor Identification. Contractor personnel must have in their possession for inspection by VA Police Department a valid state or government issued identification such as drivers license or passport. Personnel may be required to surrender their ID to the VA Police to obtain a temporary day pass at the discretion of VA Police. ID will be returned upon leaving US Government property. Place of Performance: The place of performance will be at: Department of Veterans Affairs New Jersey Healthcare System East Orange Campus GI Lab, Bld.1, 2nd Floor, 385 Tremont Avenue, East Orange, NJ 07018. Period of Performance. The base Period of Performance will be one (1) year from the date of award with the provision of four (4) Option Years.
