- Primary Care Physicians for Oregon, Washington, and Idaho. This is a Request for Information (RFI). A brief virtual Industry meeting will be available on October 31, 2019 at 10:00 am to 11:30 am Pacific Time to provide a brief overview of the procurement and to answer any questions. Place of Performance: Services shall be provided on-site at the following stations, their divisions or campuses, and their associated clinics: VA Portland Health Care System Roseburg VA Health Care System 3710 SW U.S. Veterans Hospital Road 913 NW Garden Valley Blvd. Portland, OR 97239 Roseburg, OR 97471-6513 VA Southern Oregon Rehabilitation Center VA Puget Sound Health Care System 8495 Crater Lake Hwy. 1660 S. Columbian Way White City, OR 97503 Seattle, WA 98108 Mann-Grandstaff VA Medical Center Boise VA Medical Center 4815 N. Assembly Street 500 West Fort Street Spokane, WA 99205 Boise, ID 83702 Jonathan M. Wainwright Memorial VAMC 77 Wainwright Drive Walla Walla, WA 99362 Services Required: The Contractor shall provide physicians who are Board Certified or Board Eligible in Internal Medicine to provide Primary Care Physician Services on site to beneficiaries of the Department of Veterans Affairs (VA) and the VISN 20 stations (see list above), their divisions or campuses, and their associated clinics. The estimate is that the total across all awarded contracts will be 46 Full Time Equivalent (FTE) Primary Care Physicians per year will be needed; however, that amount may be greater or less in any given year. A contractor providing onsite Primary Care Physician Services shall provide services that meet or exceed the American Board of Internal Medicine Guidelines: http://www.abim.org. General Information: It is anticipated that multiple Indefinite Delivery Indefinite Quantity (IDIQ) contracts will result from the solicitation. The number of awards will be determined after proposals have been received. All interested parties please to submit the following: Business name and address with contact information Duns and Bradstreet numbers (DUNS) Statement that your SAM is current at SAM.gov. Business size for NACIS code 561320. A capability statement detailing similar procurements performed. For all small business concerns a statement on if they plan to subcontract and if so, how they plan to comply with 13 CFR § 125.1 and 13 CFR §125.6. For Service Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) a copy of your current Veteran Information Pages (VIP) certificate. Ideally, we want all contractors to be able to provide Primary Care Physicians for all Stations. If you are unable to contract at a specific facility, please provide a list of the facilities you are unable to provide services.
