Page 3 of 3 Department of Veterans Affairs (VA) Network Contracting Office (NCO2) intends to award a sole source contract award to Olympus America, INC. 3500 Corporate Parkway, Center Valley, PA 18034-0610. The Period of Performance for Contract Award will be base period of 12 months plus 2 twelve month s options. For James J. Peters VA Medical Center, Bronx, New York 10468. NAICS: 339112 Surgical and Medical Instruments Manufacturing. Size Standard for NAICS: 1,000 Employees Product or Service Code: W065 Lease or Rental of Equipment Medical, Dental, and Veterinary Equipment and Supplies. A. Background: The James J. Peters VA Medical Center, ENT Service, is requesting to lease ENT scopes to perform procedures. B. Objectives: The objective is to lease state-of-the-art scopes that will be used as outlined below. The service agreement must make available loaners, if available, and cover all associated costs of any repairs (including shipping, parts, and labor) for the equipment outlined above through a full-service agreement, to ensure safe and reliable performance of ENT scopes at James J. Peters VAMC. Lease term pricing shall be based on a 36-month lease. C. This acquisition is conducted under the authority of the Multiple-Award Schedule Program (41 U.S.C. 251 and 40 U.S.C. 501). This is a request for approval of equipment leasing with a purchase option items in conjunction with SAC FSS items, on a delivery order under SAC contract VA119-15-A0048 for equipment lease with a purchase option of various scopes and associated equipment to be used at the James J. Peters VA Medical Center used in Bronx, NY. This procurement will be conducted in accordance with FAR Part 8. The purpose of the equipment lease with purchase option is to reduce service costs and improve equipment turn-around time and reliability. This will be a firm-fixed price delivery order contract from contract VA119-15-A0048 to include two year of option years. The following ENT scope equipment will be leased under a full-lease contract to be used in endoscopes ENT procedures: 1) ENF-VH: VH-Flexible video HD Rhino Laryngo S quantity 18 2) ENF-V4: ENF -V4 Flex slim rhinology, Pistol Gri quantity 2 3) ENF-GP2: ENF-GP2 Portable Fiber Rhinolaryngoscope quantity 10 4) PEF-V: PEF-V TRANSNASAL ESOPHAGO VIDE O SCOPE quantity 1 5) ENF-VT2: ENF-VT2 Video Theraputic Rhinolaryngosco quantity 1 6) 13839 : 13839 Aptimax Sterrad Sterilization Tray quantity 8 7) 99239 : 99239 APTIMAX STERRAD Ster. Tray Mat quantity 8 8) 99240 : 99240 APTIMAX STERRAD Ster. Tray Holder quantity 8 D. Other Requirements: This delivery order includes an option to buy all scopes at the end of the lease agreement as specified in the purchase option. THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE QUOTES; however, any firm that believes it can meet the requirements may give written notification prior to the response due date and time. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to perform the requirements. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government.
