Description
The 502d Contracting Squadron (CONS), 395 B Street W, Bldg. 224, Ste.2, Randolph AFB, TX. 78150, is issuing this Sources Sought seeking a Small Business contractor to serve as the Diabetes Self-Management Education and Support (DSMES) Subject Matter Expert (SME) in support of the 59th Medical Specialty Squadron, Diabetes Center of Excellence (DCoE), JBSA - Lackland, TX. The SME will support the DSMES Program via the Veterans' Affairs Virtual Medical Center (VAVMC) System by Recruiting, Training, and Mentoring DoD and VA Providers as Faculty/Facilitators. The position requires a Bachelor's of Science Degree in Nursing (BSN), Current Diabetes Care and Education Specialist (DCES) Certification and current Licensure by the National Certification Board for Diabetes Educators (NCBDE). NOTE: THIS IS TEMPORARY CONTRACT THAT WILL END ON 30 SEPTEMBER 2020. This Sources Sought shall not be construed as a formal solicitation or as an obligation on the part of the Government for these services. Information provided to the Government as a result of this posting is strictly voluntary, given with no expectation of compensation, and clearly provided at no cost to the Government. Interested contractors shall: 1. Provide a short synopsis (No more than 1 Page) of diabetes management experience/history, to include recruitment, training and mentoring of medical personnel. 2. Indicate if price to be offered is GSA or Open Market? _____________________ 3. Is your company a small business? ____ (Yes/ No) with a socio-economic status of ____________ (8a, HUBZone, SDVOSB, WOSB, etc.) concern. 4. The Air Force is ONLY interested, at this point, in identifying potential sources for these products as part of the overall market research effort. 5. The Government anticipates award of one contract to satisfy this requirement. A formal solicitation may be executed and posted separately at a future date. 6.
