In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. STATEMENT OF WORK COURIER SERVICES SCOPE: The contractor shall be in the business of providing courier services with properly insured/inspected company vehicles and licensed and insured drivers to transport materials and specimens from and to the Department of Veterans Affairs Medical Centers (VA), to each of the Community Based Outpatient Clinics (CBOC) Monday through Friday, except holidays and weekends. The C&A requirements do not apply, and that a Security Accreditation Package is not required. VA Medical Centers are located at: Baltimore VA Medical Center; 10 N. Green Street, Baltimore, MD 21201 Perry Point VA Medical Center; 515 Broad Street, Perry Point, MD 21902 Loch Raven VA Medical Center; 3900 Loch Raven Blvd, Baltimore, MD 21218 CBOCs are located at: Cambridge CBOC; 830 Chesapeake Drive, Cambridge, MD 21613 Pocomoke City CBOC; 1701 Market Street, Unit 211, Pocomoke, MD 21851 Rosedale CBOC; 5235 King Avenue Franklin Square Professional Center Suite 200, Rosedale, MD 21237 Fort Meade CBOC; 2479 5th Street, Fort Meade, MD 20755 Glen Burnie CBOC; 808 Landmark Drive Suite 127, Glen Burnie, MD 21061 ITEMS-TO BE TRANSPORTED: Specimen containers for delivery/pickup are approximately 15 inches by15 inches by 20 inches in size and sometimes additional smaller containers for "room temperature" specimens or paperwork. Laboratory specimens, i.e. body fluids, and human blood products, will be properly packaged by the VA laboratory staff and CBOC staff for transportation and will be placed in proper shipping containers weighing approximately l 0-20 pounds; and 18 inches by 18 inches by 18 inches in size, (estimated) Instrument transport containers for delivery/pickup are approximately 15 inches by15 inches by 20 inches in size, red in-color and sometimes smaller containers for transportation and will be weighing approximately 10-20 pounds (estimated). Occasional supply transport containers for delivery/pickup are approximately 10 inches by12 inches by 20 inches in size and will be weighing approximately 10-20 pounds (estimated) Contractor will check with the VA (originating facility) for transport items prior to beginning the round trip to the CBOC facilities. PICK UP AND DELIVERY: A. EASTERN SHORE: 1. Pickups at the CBOC facilities shall be within thirty (30) minutes of scheduled time. A delivery is considered late after thirty (30) minutes of scheduled time. Pickup at the CBOC facilities are listed below: Cambridge CBOC: Monday - 1:00pm for delivery to Perry Point, MD Tuesday- Friday; 08:00am and 12:00pm for delivery to Perry Point, MD Pocomoke City CBOC: Monday 1:00pm for delivery to VAMC, Perry Point, MD Tuesday- Friday; 09:00am and 1:00pm for delivery to Perry Point, MD 2. Deliveries from the CBOCs to the VA will be between 8:00am to 5:00pm. Deliveries will be made directly to the Laboratory Service at the VAMC, Perry Point B. GREATER BALTIMORE: 1. Pickups at the CBOC facilities shall be within thirty (30) minutes of scheduled time. A delivery is considered late after thirty (30) minutes of scheduled time. Pickup at the CBOC facilities are listed below: Rosedale CBOC: Monday Friday; 8:00am, 12:00pm, 4:00pm for delivery to VAMC Baltimore, MD Glen Burnie CBOC: Monday Friday; 8:00am, 12:00pm, 4:00pm for delivery to VAMC Baltimore, MD Fort Meade CBOC: Monday Friday; 8:00am, 12:00pm, 4:00pm for delivery to VAMC Baltimore, MD 2. Laboratory specimen deliveries from the CBOCs to the VA will be between 8:00am to 5:00pm. Deliveries will be made directly to the Laboratory Service at the VAMC, Baltimore, MD 4th Floor 3. Instrument delivery from CBOCs to the VA will be between 8:00am to 5:00pm. Deliveries will be made directly to Sterile Processing Service at the VAMC, Baltimore, MD Basement C. LOCH RAVEN MEDICAL: 1. Pickup at the Loch Raven facilities shall be within thirty (30) minutes of scheduled time. A delivery is considered late after thirty (30) minutes of scheduled time. Pickup at the VAMC Loch Raven is listed below: Loch Raven Clinic: Monday Friday; 8:00am, 12:00pm, 4:00pm for delivery to VAMC Baltimore, MD Loch Raven CLC: Monday Friday; 7:30am, 12:00pm, 4:30pm for delivery to VAMC Baltimore, MD 2. Laboratory specimen deliveries from the CBOCs to the VA will be between 8:00am to 5:00pm. Deliveries will be made directly to the Laboratory Service at the VAMC, Baltimore, MD 4th Floor 3. Instrument delivery from CBOCs to the VA will be between 8:00am to 5:00pm. Deliveries will be made directly to Sterile Processing Service at the VAMC, Baltimore, MD Basement D. CHANGES: The VA reserves the right to negotiate additional routes or changes to existing routes in accordance with the "CHANGES" clause of this contract without further competition. We currently do not have plans to add additional routes but as Veterans demographics change, the current route may need to change. SAFETY: The contractor is responsible for the safe transportation of all items to their destination. The contractor shall insure that all drivers are trained and have received instructions in the order handling of items to be transported under this contract. This shall include instructions on procedures to following in the event of leaks, spills, or breakage of packaging for the lab specimens. DRIVERS: All drivers must maintain a valid driver's license for the type of vehicle operated as required by the State of Maryland. A record of each driver as to character and physical capabilities of performing the duties as a courier must be maintained and made available to the Contracting Officer for inspection upon request. VEHICLES: The vehicles used in the performance of this contract must be licensed and meet the minimum requirements as mandated by the State of Maryland. Vehicles must provide protection for the items being transported to prevent exposure to weather (direct sunlight, heat, rain, snow, water, etc., and insure items are secured to prevent theft or loss.
