We will be happy to assist them in getting the correct answers and then will make minutes and send them out so everyone will have the same information. Lessors will send all questions through Contracting; Contracting will coordinate with the COR to have the appropriate answer as quickly as possible and distributed to all interested parties. Minutes Action Items and Follow Up # Item Responsible Due 1 I am emailing to confirm that the V.A.'s intended setback requirement is actually 50 feet? We normally see this setback at 25 feet for CBOCs, so I just want to confirm 50 feet is the correct size requirement. FYI - the source we are using for the 50 ft setback requirement comes from "Exhibit B" of the RLP document set, labeled "Additional Information". Please see page 1 of that document under "Parking Area Safety"...the last bullet point in that section reads "Vendor will not allow any vehicle parking at least 50 feet from the Building". 50 feet setback is correct for vehicle parking. 25 feet setback is for ambulance drive up. None 2 We would like to confirm that the V.A. requires a "loading dock and freight elevator" per the RLP requirement listed on Page 3 (Item "M") of the RLP. No loading dock or freight elevator is required None 3 How many separate Form 1364's are you asking us to submit (3 or 5?) given the language on Page 2 (Item "F") of the RLP which references pricing options for scenario A, B, and C? That is, one could interpret we are required to submit two separate 1364's just for scenario B. 3 are required: Alternative A) 20-year firm term; Alternative B) 15-year firm term, with one 5-year option, and Alternative C) 10-year firm term, with one 10-year option. None 4 Exhibit P "Operation and Maintenance Plan" appears to have been omitted from the RLP document set, unless there is no form document and you are just asking us to submit a Plan without a template? Operation and Maintenance Plan Attached None 5 Does the V.A. intend to pay the T.I. award money upfront or rather, amortize it over the term of the lease? Lease No 36C25019L0012 Section 1.07 B. The Government shall have the right to make a lump sum payments None 6 Do you want us to supply built in or a modulated desk for the receptionist? Built in casework. None 7 Is the V.A. requiring a hearing aid booth or just a hearing aid fitting room? Hearing aid fitting room only None 8 Is the Green Globes certification required? If so, is a Level I or Level II cert. required? Lease Section 4.05 J. Green Globe I certificate is required. Only one J para should be there. LEED does not apply None 9 Do we supply an ice machine or does the V.A. provide? No ice machine will be installed None 10 Do we need to provide internet service / cable tv service? Internet service is required in the Lobby, the VA will provide services to our Network None Is a Phase I Environmental Study required? If area is a new build and after submission proposal, a Phase I can be directed by the LCO. None
