This RFI is issued solely for information and planning purposes it does not constitute a Request for Quote (RFQ) or a promise to issue an RFQ in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, the Department of Veteran Affairs is not at this time seeking proposals and will not accept unsolicited proposals. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party s expense. Not responding to this RFI does not preclude participation in any future RFQ or RFP, if any is issued. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. The Government is seeking Veteran Owned Small Businesses ONLY who can provide the following equipment for the TOMAH VA Medical Center in TOMAH WI: SPECIFICATION/STATEMENT OF WORK LEASE/RENTAL OF TOSHIBA AQUILION 64 SLICE CT SCANNER WITH A CT INJECTOR IN A PARKED MOBILE TRAILER TOMAH VAMC The purpose of this contract is to provide the Tomah VA Medical Center (VAMC) with a three month lease with the option to rent per month, if required, thereafter, up to but not exceeding three additional months of a mobile Computer Tomography (CT) scanner. SCOPE Toshiba Aquilion 64 Slice CT scanner with a CT Injector, or approved equal, in a Parked Mobile Trailer. Lease shall last three (3) consecutive months. See period of performance for defined months of need. The Contract should allow the option to rent this equipment month by month thereafter not to exceed three (3) additional months. Vendor will be responsible for mobilization and demobilization, initial set up of CT scanner and CT injector and all connections of Mobile CT scanner and related equipment to existing onsite data and power. The Contractor shall provide the following: Toshiba Aquilion 64 Whole Body Computerized Tomography Scanner: Large-aperture slip-ring gantry, extra-wide couch, ergonomic operator console, high-frequency X-ray generator, high-heat-capacity X-ray tube Aquilion Gantry: Aperture: 72 cm, Five scan fields of view: 18, 24, 32, 40 and 50 cm, Gantry controls on both sides, patient positioning lights, scan times of 0.4, 0.5, 0.75, 1, 1.5, 2 and 3 seconds full rotation Couch: 47cm wide, metal-free couch top, dimensions of 2,190 mm wide and 1,800 mm long, manual control of table increments by using typical protocol from the gantry and console, supports 450 lbs Console: Keyboard, mouse, monitors, CPU cabinets, power controls, image display, scanogram control, couch-top remote control, gantry tilt control, window level and width adjustment, three preset windows can be stored in typical exam plans, other mouse-operated, image-processing functions, high line-rate, 19-inch LCD monitors, displays images in 512x512 or 1024x1024, CT number display ranges from -1,536 to +8,191 and programmable voice commands GOVERNMENT FURNISHED EQUIPMENT AND SERVICES CT Technologist and Support Staff Pad for Parking Scanner with Tractor/Trailer access 480v 3ph 150A Electrical Power w/ either Russell Stoll, Appleton or Crouse Hinds Connection Any Required Telephone / Network Connections All Useable or Consumable Goods (film, contrast, linen, etc.) Radiologist who will Provide General Supervision and Patient Care Scheduling, Delivery and Retrieval of Patients and Safety Code Compliance GENERAL INFORMATION It is strongly suggested, that the contractor inspect the site during the solicitation, to fully understand the conditions under which the Mobile CT Scanner Trailer will be parked at the facility of Tomah VAMC. Contractor is responsible to replace any items damaged by the contractor, sub-contractor, their employees or equipment at no extra charge to the Government. Contractor shall not interrupt any utility service while performing under this contract. Contractor shall coordinate with COR at the time of Mobile CT Scanner mobilization and connections to Tomah VAMC Facility infrastructure. i.e. power cord, data, etc. SECURITY REQUIREMENTS Paper, plastic or other similar based media containing VA sensitive data that is not sent to the VA will be properly disposed of by methods such as shredders with no larger than 1/8 inch width cuts and then cross cut. This media will be destroyed such that information may not be retrieved. Media with small print, such as microfilm will be completely destroyed such as to render the information unrecoverable. The contractor will take due diligence to make sure that VA sensitive information and data that is viewed, faxed or similarly transmitted, or discussed verbally is protected from unapproved disclosure. VA sensitive information and data may not be transmitted across the Internet unencrypted (including email and instant messaging) and must be protected by (VA-VPN) VA Virtual Private Network and/or VA approved encryption process (Example: PKI - Public Key Infrastructure). VA sensitive information may not reside on non-VA systems or devices unless specifically designated and approved as appropriate for the terms of the contract. All systems that store or process VA data will be protected with VA approved encryption (typically FISPS 140-2 compliant). Any security violations or suspected violations shall be immediately reported to the VA Contracting Officer and the assigned VA Information Security Officer (ISO).
