Amendment #2 RFQ 36C25919Q0516 The purpose of this amendment is to add the following: Change the schedule (v) to - ITEM # DESCRIPTION OF SUPPLIES/SERVICES 0001 Off-Site Lodging /Including Shuttle Service Period of Performance 11/1/19 thru 10/31/2020 **Note: provide daily room rate per hotel offered Daily Rate Annual Total 1001 Off-Site Lodging /Including Shuttle Service Period of Performance 11/1/20 thru 10/31/2021 **Note: provide daily room rate per hotel offered Daily Rate Annual Total 2001 Off-Site Lodging /Including Shuttle Service Period of Performance 11/1/21 thru 10/31/2022 **Note: provide daily room rate per hotel offered Daily Rate Annual Total 3001 Off-Site Lodging /Including Shuttle Service Period of Performance 11/1/22 thru 10/31/2023 **Note: provide daily room rate per hotel offered Daily Rate Annual Total 4001 Off-Site Lodging /Including Shuttle Service Period of Performance 11/1/23 thru 10/31/2024 **Note: provide daily room rate per hotel offered Daily Rate Annual Total Total 5-year Add 2.f to the statement of work 2.f Contractor shall provide hotel/lodging facilities with a minimum of 4 diamond or 4-star rating from independent originations such as AAA and/or Trip Advisor. Removes 5.d If a Veteran cancels their VA appointment, but fails to cancel lodging within the specified hotel/motel timeframe, there will be a charge to the Veteran credit card. Vouchers are given to the veteran and the Medical Center makes the reservation. Change (viii) 52.212-1(g) to Volume I- Technical:   The offeror shall submit a completed Attachment 1, List of Requirements, which details all the technical aspects listed and identified in the Statement of Work. Volume II Price: The offeror shall complete the Price Schedule of Supplies/Services of this solicitation, with proposed line item prices inserted in appropriate spaces and detail/itemize the pricing or add additional line items, if applicable. The Government requires contractors to quote unit prices and total prices that are two (2) decimal places. The unit price multiplied by unit quantity must equal the total price for that line item. Change (ix) 52.212-2(a) to Technical: The Government will evaluate the responses provided to Attachment 1, List of Requirements.   If an offeror marks no to a minimum of one (1) requirement listed therein, the offeror s quote will be excluded from further evaluation and no longer considered for award. The technical rating is on the below: Table A Technical Ratings Rating Description Acceptable The technical volume clearly meets the minimum requirements of the solicitation. Unacceptable The technical volume does not clearly meet the minimum requirements of the solicitation. Price Initially quotes shall be ranked according to price, including option prices. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offers that do not meet or exceed the requirements of the Statement of Work shall not be selected regardless of price. NOTE: In accordance with FAR 52.217-8 Option to Extend Services, half of the price (6 months) of the last option year will be added to the price and evaluated upfront in the event this clause is exercised. However, the additional 6-month pricing will not be included in the pricing notification to all offerors. Attachment 1 List of Requirements Technical Requirements Yes No Section 2.b Have a minimum 3 diamond or 3-star rating from independent originations such as AAA and/ or Trip Advisor Section 3.1.a A minimum of fifty-six (56) rooms per day are required to be reserved for VA veterans. Five (5) of which need to be American with Disabilities Act (ADA) compliant. Section 3.1.b Lodging within 10-mile radius location of the VAMC Section 3.1.c Rooms shall be available Sundays thru Friday. (Checkout Saturday) Section 3.1.d All rooms offered shall be non-smoking rooms. Section 3.1.e Rooms should have two beds to accommodate a care giver as needed. Section 3.1.f Rooms shall be fully furnished: Bed(s), Telephone, TV, and Refrigerator. Section 3.2.a A minimum of thirty (30) rooms per day are required to be reserved for VA veterans. Section 3.2.b Hotel shall be within 15 miles of the Medical Center. Section 3.2.c Hotel shall be an extended stay accommodation. Transplant patients are accompanied by a support person and require lodging until medically released back to their home state or other VA Medical Center. The average stay is between 1 week and 18 months. Rooms should have two beds to accommodate a care giver as needed. Section 3.2.d Rooms shall be available Sundays thru Saturday. Section 3.2.e All rooms offered shall be non-smoking rooms. Section 3.2.f Rooms shall be fully furnished: at least two beds, Telephone, TV, and fully equipped kitchen/kitchenette. Laundry facility required. Section 3.3 All bids should list any/all meals provided by the hotel/lodging. State whether they are free or the cost, which meal, and times available Section 5.a The Contractor shall guarantee the Medical Center that there shall be no Blackout Dates on any reserved rooms. Seasonal rate changes are not allowed. Section 5.b All hotel industry standards for health and safety must be met or exceeded. NFPA, ADA, OSHA and all city, county, state ordinances/codes. This shall include all housekeeping regulations. Section 7.a Shuttle service to and from the airport, the VA Medical Center, and the University of Utah Medical Center is required for transplant patients from the extended stay lodging(s). Hours of operation from 6:30 a.m. to 6:30 p.m. ADA compliant if possible. Section 7.b Shuttle service to and from the VA Medical Center and airport for standard patients is recommended. Hours of operation from 6:30 a.m. to 6:30 p.m. ADA compliant if possible.
