- This Amendment is to answer questions submitted by contractors. Is there a Statement of Work for this requirement? A Performance Work Statement is below Is the contractor required to be licensed in the state of California by the contract performance date? In accordance with VAAR 852.237-70, the contractor shall obtain all necessary licenses and/or permits required to perform the work. (see the full VAAR 852.237-70 clause on page 19 of the solicitation) Is there currently telemetric monitoring for this tank system? Yes, there is currently telemetric monitoring for the tank system. How will contractors bill for Emergency Deliveries? Contractors will bill for emergency deliveries through www.tungsten-network.com. Are there any delivery window or day of the week restrictions? Delivery hours are 8 pm 4 am, Mon Fri, 6 pm 6 am, Sat Sun. Is award based on Lowest Price Technically Acceptable? Award will be made to the Lowest Price Technically Acceptable offer that is considered price reasonable. Confirm the Tank Sizes for the Main & Reserve (in Gallons) The main tank has a 6,000 gallon capacity. The reserve tank has a 525-gallon capacity. Confirm Annual estimate usage The estimated usage is 120,492.90 CCF per year. PERFORMANCE WORK STATEMENT SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary provide medical grade quality bulk oxygen for patient care to the VA San Diego Central Plant, Building 2. Provide bulk liquid oxygen product, inspection, calibration, annual certifications of all gauges and alarms. Include all maintenance upon request. Oxygen product per NAC, NFPA 99, & TJC. 1. AREAS TO BE MAINTAINED: Building 2 Bulk Oxygen storage facility including de-icing as required. 2. TASK FREQUENCY AND INSTRUCTIONS: Fill oxygen tanks with medical grade oxygen as needed provide telemetry for automatic refill notifications and/or remote alarms. Liquid Oxygen handling must be in accordance with NFPA & CGA. Maintain and repair all systems on the bulk oxygen pad including de-icing as necessary. Test and certify alarm systems annually. 3. SPECIAL WORK REQUIREMENTS: a. Upon the delivery of oxygen supply, maintenance or repair, contractor shall provide a signed, legible, written report with The Central Plant Supervisor indicating the date and what work was completed during the day. The report for deliveries will include quantity, quality of product, time in, time out and signed all reports will indicate current status of the Oxygen supply and delivery system. Prior to departing the station, all bulk oxygen system components will be checked for proper functioning. The report format will be approved by the government. If no form is left, it will indicate that no work was accomplished, and default or liquidated damages clauses shall be invoked. b. QUALITY OF SERVICE: The contractor shall have competent, documented experience as a medical grade oxygen maintenance and repair system per NFPA & CGA. 4. PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. 5. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall always wear visible identification while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate desig nated park ing areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking viola tions of the con tractor under any con ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two-way radios are not to be used within six feet of any medical equipment. Posses sion of weapons is prohibited. Enclosed containers, including tool kits, shall be sub ject to search. Violations of VA regulations may result in a citation answer able in the United States (Federal) Dis trict Court, not a local district state, or municipal court.
