Description
Statement of Work Counselor Unity Healing Center Nashville Area Office-Indian Health Service 448 Sequoyah Trail Cherokee, NC 28719 Statement of Work Unity Healing Center is a 16 bed residential facility that provides a multi-disciplinary, multi-model (culturally relevant and traditional therapy) substance abuse treatment program for Native American adolescents. Provides substance abuse counseling for residents with primary diagnosis of substance use dependence. Scope of Work · provide direct services including crisis intervention, assessments, treatment planning and management for individual, family and group psychotherapy for adolescent residents under the supervision of the Clinic Supervisor · complete and maintain all appropriate resident records including intake documents, assessments, treatment plans, discharge summaries, and progress notes, collaborate with medical staff and other clinical team members, complete documentation as required by UHC policies and guidelines; · consult with school, legal, and community partners; · complete follow-up activities with resident, families, and referring agents; and · provide supervision of licensed staff, including ensuring job duties are completed · participates in team meetings and facility trainings · participates in and/or initiates promotional, educational, preventive, or advocacy efforts in the community on behalf of mental health needs of residents and promotes a recovery lifestyle. · Contributes to program planning, development and evaluation of clinical services through active participation in performance improvement activities. · Maintain smoke-free facility · Services are provided seven days a week. · Work may be required on any shift to include weekends and holidays. · Occasional travel for training and outreach is required. · Provide individual and group counseling, case management, assessment, and aftercare to adolescents in residential treatment facility. · Experience or certification/licensure in substance abuse required. · Knowledge of Matrix Model and providing services for co-occurring disorders required · Knowledge and skills in the diagnosis a treatment of individuals, families and groups of people with acute and chronic mental health problems, specifically substance use disorders, within an adolescent residential setting. · Knowledge of psychological disorders as defined by the Diagnostic and Statistical Manual of Mental Disorders (DSM 5) · Knowledge of the socio-cultural factors that affect the mental health of Native American populations and skills in the development of treatment planning that can ameliorate the problems. Ability to communicate and cooperation with organizations and motivate available community resources. · Knowledgeable of Joint Commission guidelines and standards, as well as Federal and State laws particularly in the area of confidentiality of records and protected health information. Compliance with professional regulations and with the professional Code of Ethics for the respective discipline is also required. · Work is mostly sedentary, involving long periods of sitting while interviewing and counseling residents. Some walking in the facility and on the grounds. Some lifting of carrying of light items. Dealing with residents’ interpersonal problems adds extensive periods with high emotional demand and stress. The work involves possible exposure to physical hazards and harm from violent and unruly residents. Some outdoor experiences with residents such as hiking, walking, and other recreation activities require attendance to allow residents to process the experience and reflect with counselor.
