Page 13 of 13 Combined Synopsis Solicitation: Police Radio Dispatch Console General: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is 36C25019Q1480 and is issued as a Request for Quote (RFQ). This solicitation document incorporates provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-06, effective September 10, 2019. This procurement is a 100% small business set-aside and only qualified contractors may submit quotes. This requirement will be awarded on all-or-none basis and is Brand Name Only. The applicable North American Industry Classification System Code (NAICS) is 339113 and the small business size standard is 750 employees. Line items: Brand Name ONLY Manufacturer Guldmann, Inc. Weight tested and certified LOCAL STOCK NUMBER: GH3+ 825 w/scale GRAND TOTAL Statement of Work A. Background. Contractor shall provide all labor and materials to perform site preparation and install five (5) permanent ceiling and/or wall mounted patient care lifts for the Women s Health Center (Learning Exchange Building Addition). In patient rooms, three of the ceiling lifts require integrated scales to weigh dependent patients both bariatric and non-bariatric. In bathrooms, two of the ceiling lifts do not require integrated scales as they will be used to address dependent patient s toileting needs. All work shall be in direct compliance in accordance with the VHA Safe Patient Handling and Mobility Directive 1611 (3/28/2018) (Attachment 1), as well as the Medical Center s policy on Safe Patient Handling & Mobility Program (MC 118-034 dated 1/2/2016) (Attachment 2). B. Scope. The Contractor shall provide the lifts to the CLE VAMC. C. Specific Tasks. Site Preparation and Installation for five (5) EA GH3+825 systems in rooms listed below: Site Preparation shall comprise of all structural modifications to facilitate lift installation.   The contractor will also be responsible for removal of all material required for equipment installation. See attached drawings for specific locations. Installation shall be comprised of; required lift structural support systems to install proposed equipment. Three (3) EA GH3+825 w/scale systems Women s Health Center-Located in the Learning Exchange Building Addition Women s Health Center Rooms requiring ceiling lifts w/scales L881 (1) L845 (1) L890 (1) Two (2) EA GH3+825 wo/scale systems Women s Health Center-Located in the Learning Exchange Building Addition. Women s Health Center Rooms requiring ceiling lifts w/o scales L881A L845A Provide two (2) Brand Name Guldmann GH3+825 wo/scale ceiling lift systems. Provide three (3) Brand Name Guldmann GH3+825 w/scale ceiling lift systems (Note: Each handset must have an integrated scale for patient weight into the handset controller without compromising bed to hangar bar height.) Contractor will connect transformer/ power supply to VA supplied 120Volt power source. Place of Performance. Louis Stokes Cleveland VAMC 10701 East Blvd. Cleveland, Ohio 44106 E. Period of Performance. ARO 180 Days Government Holidays. Offer Evaluation and Award. This is a competitive Request for Quotations (RFQ) that will be evaluated under FAR Part 12 and 13. The Government will issue a Firm-Fixed Price, Brand Name Only, Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the requirements within this solicitation and will be the most advantageous to the Government. Lowest Price Technically Available will be how the quotes are evaluated. This is a Brand Name Only solicitation. An Offeror s initial quote shall contain the best terms from a price and technical standpoint. Offerors are reminded that representatives from your company SHALL NOT contact any CLE VAMC personnel to discuss this RFQ during the solicitation and evaluation process. 6. Procedure for submitting documents to the Department of Veterans Affairs. DOCUMENTS THAT ARE TO BE PROVIDED TO THE CONTRACTING OFFICER WILL BE SUBMITTED USING THE APPROPRIATE VEHICLE BELOW. email: rachelle.hamer@va.gov 7. Submission of Offer. All information shall be submitted in the manner described below and offerors who do not submit all information may not be considered for award. Set-Aside Requirement: Only verified Small Business firms are eligible to submit an offer or receive an award. This is a Brand Name Only solicitation. Contractor shall provide an authorized distribution letter. The Contractor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. The Contractor shall provide new items ONLY; no remanufactured or gray market items. All items must be covered by the manufacturer s warranty.   Quote must be good for 30 calendar days after close of this Combined Synopsis Solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
