Statement of Work Construction Dumpster and Cardboard compactor Disposal Beckley Veterans Affairs Medical Center (VAMC) September 6, 2019 1. GENERAL The contractor shall furnish all personnel, equipment, and transportation required to dispose of construction debris and recyclable cardboard located at the Beckley Veterans Affairs Medical Center in Beckley, WV. Services shall be performed on-site. The contract for the 30-yard dumpster and the Cardboard compactor is based off Beckley VAMC past orders. We estimated 26 pulls every year every two weeks, and a total estimated tonnage for the 30 Yard Dumpster of 85 tons a year. The pricing schedule is as follows: 30 Yard Dumpster Rental Fee Per Month 12 Months 30 Yard Dumpster Haul Fee Per Haul 26 Hauls Per Year 30 Yard Dumpster Landfill Fee Per Ton 85 Tons Per Year Cardboard Compactor Rental Fee Per Month 12 Months Cardboard Compactor Haul Fee Per Haul 26 Hauls Per Year Cardboard Compactor Landfill Fee if any, most are free but contain fees for contamination. 2. SCOPE Requested Base Plus 4 Option Period Contract. Base Period 10/01/2019 09/30/2020 Option Period 1 10/01/2020 09/30/2021 Option Period 2 10/01/2021 09/30/2022 Option Period 3 10/01/2022 09/30/2023 Option Period 4 10/01/2023 09/30/2024 2.1 The contractor shall provide one cardboard compactor and one 30-yard construction dumpster for the disposal of construction debris and recyclable cardboard. The dumpster and compactor shall be located at the back-loading dock of bldg. 1. 2.2 The contractor shall remove the construction dumpster and compactor from Beckley VAMC and dispose of solid waste at a West Virginia approved land fill. Construction debris and cardboard removal shall be completed bi-weekly and or within 24 hours of notification by the COR. 2.3 Upon return of the dumpster and compactor, the contractor is responsible for ensuring that it is rinsed and free of odor, and that the surrounding area is cleared of any debris. 3. COORDINATION Before contract performance, the contractor shall provide documentation of a West Virginia approved landfill. Documentation shall be provided annually for all subsequent option years. 4. APPLICABLE REGULATIONS, GUIDELINES AND STANDARD OPERATING PROCEDURES Solid waste and cardboard shall be removed in accordance with the Environmental Protection Agency, the West Virginia Solid Waste Program and the West Virginia Solid Waste Authority. 5. CONTRACTOR IDENTIFICATION While on Beckley VAMC premises, contractor employees will wear a company uniform and a visible identification badge which contains a photograph of the employee. All contractor vehicles and the dumpster and compactor shall be clearly identified with the company s name. 6. WORK HOURS: Work shall be accomplished Monday through Friday, 8:00 am to 4:30 pm. Unless directed by the Contracting Officer (CO), work at the site(s) shall not take place on Federal holidays or weekends. When a holiday falls on Sunday, the following Monday will be observed as a legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed by the U.S. Government agencies. The following Federal holidays are observed: New Year's Martin Luther King Jr Day President's Day Memorial Day Fourth of July Labor Day Columbus Day Veteran's Day Thanksgiving Christmas This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127. TIERED EVALUATIONS INCLUDING SMALL BUSINESS CONCERNS: This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alternative, a tiered evaluation for VOSB concerns, or in the alternative, a set-aside for other small business concerns with HUBZone small business concerns and 8(a) participants having priority. If award cannot be made, the solicitation will be cancelled and the requirement resolicited.
