Only One Responsible Source for the requested brand name equipment and availability of rental periods, will be supplied by one distributor. The associated NAICS code for this is 531120. The vendor selected will be Shared Medical Equipment Group, 209 Limestone Pass, Cottage Grove, WI 53527. Description of Equipment Rental: Item # Qty Description 1 1 Siemens Sensation 64 Slice CT Scanner Mobile Trailer for 11/6/19 to 12/18/19 2 1 Siemens Aera 1.5T MRI Mobile Trailer for 1/6/20 to 2/28/20 A firm fixed price best value price contract with no option years is anticipated and shall be awarded.
