- Contractor Submittal Form (should be submitted with as the quote) Performance Work Statement (PWS) Combo 1. The contractor shall provide non-personal services to provide all labor, parts, tools, materials, equipment, personnel, supervision, transportation, and other items or services necessary to remove and install blinds and appliances for dormitory 590, in a manner that will ensure continuous and reliable operation in accordance with the PWS. 2. The anticipated period of performance is 60 days after award at Peterson Air Force Base, CO. Please submit an estimated schedule with your quote. The schedule shall include estimated lead times for parts and materials along with the time needed to complete the project at the site location. 3. The quote must include a signed statement by a person authorized to obligate the company contractually, stating that the quote and prices submitted will remain in effect for 30 calendar days after the solicitation closing. This statement may be added as text to email when submitting the quote. 4. All questions regarding this RFQ must be submitted in writing to Erica Dixon at erica.dixon.7@us.af.mil AND TSgt James Pitcher at james.pitcher@us.af.mil NLT 1:00 P.M. MST 16 Sep 2019. 5. Your quote with all required documents must be received NLT 1:00 P.M. MST 19 Sep 2019. Quotes received after that time may not be considered for award. Submit your quote via email to Erica Dixon at erica.dixon.7@us.af.mil AND TSgt James Pitcher at james.pitcher@us.af.mil. Please include a product sheet for CLIN 0001, CLIN 0002, and CLIN 0003.
