Exhibit C Amended 13 Sept 2019 RFP W9124219R0002 Amended 13 Sept 2019 Q. Please post a "clean" SF1449; current one says "DRAFT." A. See SF1449 posted to FBO.gov ____________________________________________________________________________________ Q. As NCARNG issued a Sources Sought in December 2018 and likely at least two WOSB responded, will you change the set-aside to WOSB or Total Small Business with NAICS 621112, 621399, or 62199 to assure legitimate small business can compete with a revenue ceiling more advantageous to small business? A. Suitability of NAICS selection in relation to the scope of work was vetted through SBA Procurement Center Representative and NGB Director of Small Business Programs ____________________________________________________________________________________ Q. Of the 28-32 events per year, how many of those are run concurrently? What is the largest number of concurrent locations on any given weekend? Historically, how many weekends per year do those 28-32 events occur? A. With respect to PHA Events, 6 Weekends historically have more than 1 Medical readiness Event Scheduled. One Weekend has historically had 4 Medical Readiness Events Scheduled; see PWS Technical Exhibit 3 ____________________________________________________________________________________ Q. Are the six (6) pre and post-deployment events separate or concurrent events? Are they included as part of the 28-32 yearly events? A. The six (6) pre and post-deployment events are generally run concurrently and are included in the historical 28-32 yearly events; see PWS Technical Exhibit 3 ____________________________________________________________________________________ Q. Regarding Bid Schedule, pg. 8 and all line items saying "...PHA Event (Dental excluded) hosted @ Gov't Facility:" Is this indicating all medical services can be performed inside the facility? If not, what of the medical is excluded from being done in Gov't Facility? A. All but hearing may be completed in the hosted Gov't Facility; see PWS para 3.1.1 ____________________________________________________________________________________ Q. Regarding Bid Schedule, pg. 8 and all line items saying "...PHA Event w/Dental Services hosted @ Gov't Facility:" Is this indicating all medical and dental services can be performed inside the facility? If not, what the medical and dental is excluded from being done in Gov't Facility? A. All but hearing may be completed in the hosted Gov't Facility; see PWS para 3.1.2 ____________________________________________________________________________________ Q. Regarding PWS para 5.1.10.8, may self-contained portable dental units be used for dental treatment within Gov't Facilities? A. Self-contained portable dental units may be used for dental treatment within Gov't Facilities; see PWS para 5.1.10.8 ____________________________________________________________________________________ Q. Regarding Bid Schedule, pg. 8 and all line times saying "...Fully Mobile:" Will any space be available within Gov't Facility for administrative functions (check in/out, etc...)? A. If it is required Administrative Functions may be completed inside, see PWS para 3.1.3 ____________________________________________________________________________________ Q. Regarding PWS para 5.1.10.2 - Soldiers scoring worse than 20/40 need to see Optometrist. Is the 20/40 corrected or uncorrected vision? A. 20/40 in uncorrected vision; see PWS para 5.1.10.2 ____________________________________________________________________________________ Q. Regarding 5.1.10.5 - When is a PSA, Sickledex, Hematocrit or HgB indicated at a PHA/SRP or are these Occ Health labs, only? A. PHA Event Phlebotomy Services are identified in PWS para 5.1.10.5 and Occupational Health Event Phlebotomy Services are identified in PWS 5.1.11.4. ____________________________________________________________________________________ Q. No mention is made in PWS for SPRINT testing on H3 SMs. Will SPRINT testing be done by NCARNG or contractor? A. When a Comprehensive Audiology Exam identifies the Soldier as a true H3, the Contractor will perform a Speech Recognition in Noise Test (SPRINT) by a licensed audiologist; see PWS para 5.1.10.3 and 5.1.11.2 ____________________________________________________________________________________ Q. Are DOEHRS uploads being done by NCARNG or contractor (to include Hearing Screenings)? A. DOEHRS, to include hearing screening will be uploaded by Contractor; see PWS para 5.1.1 ____________________________________________________________________________________ Q. Regarding walk-in and additions to the roster once final scrub is complete- will there be a limit to the number of walk-ins permitted at an event? A. Walk-ins are sometimes experienced (unexpected soldiers at events). Walk-ins in excess of 10% of the final contracted quantity of soldiers contracted for at any single event, will be compensated at the daily soldier rate for any single awarded task order.; see PWS para 5.4 ____________________________________________________________________________________ Q. Regarding Key Personnel - Resumes of Key Personnel would significantly take up the 15-page limit in Sub-factor one. Will you consider eliminating the submission of resumes until post-award, or exclude them from the 15-page limit as you are for Exhibit A and B? A. Documentation of Key Personnel are excluded from the 15 page limit in Subfactor One; see addendum to 52.212-1 ____________________________________________________________________________________ Q. Page 80 at 2. Proposal Files says: VOLUME II, PART 1, TECHNICAL PROPOSAL. Should this be Volume I? A. Yes. ____________________________________________________________________________________ Q. Regarding Exhibit C: If a major subcontractor is asked to complete this, are they the "Offeror" at A., or should they identify themselves as our subcontractor?? Do they need to do Part 2? A. Exhibit C has been updated. ____________________________________________________________________________________ Q. Are any of the Occupational Health events conducted concurrent to one another? A. No ____________________________________________________________________________________ Q. Regarding PWS para 5.2.6, does Occ Med Physician have to be onsite or can they view exams and test results off site? A. Does not have to be onsite, can review exams/results off site; see PWS para 5.11 ____________________________________________________________________________________ Q. Can a provider other than the Occ Med Physician do the hands-on physical portion? A. Yes, a licensed Physician Assistant or licensed Nurse Practitioner can also perform the hands-on physical portion; see PWS para 5.2.7 Licensed Physician Assistant or Licensed Nurse Practitioner: Contractor may provide a Licensed Physician Assistant or Licensed Nurse Practitioner to assist in performing physicals. ____________________________________________________________________________________ Q. Historically, how many technicians have needed each of the different test panels? (list numbers for each panel) A. With majority personnel present (not deployed), average quantity for each test: Work/Medical History Forms - 370 Height/Weight/Blood Pressure - 370 Audiogram - 110 Blood (Chemistry Panel) - 360 Blood (Lead/ZPP) - 30 Pulmonary Function Test - 45 Respirator Clearance - 45 see PWS Technical Exhibit 3 ____________________________________________________________________________________ Q. The Occ Health Estimated Workload Data shows twelve days of service. Can contractor complete the events in less days than indicated for each site? A. The events may be executed in less than twelve days; see Bid Schedule language ____________________________________________________________________________________ Q. Can any portion of the Occ Heath exams be done within the government facility? A. For multiple reasons, all portions of the Occ Health exams are to be completed in one area fully mobile, not within the government facility see PWS para 4.1.1 and the Bid Schedule
