Pecos Road, Building 6 North Las Vegas, NV 89086 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, and Part 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ), and is posted as 100% Set-Aside for small business sources. The Department of Veteran Affairs Northern California Healthcare System is intending to award a Firm Fixed Price (FFP) contract as a result of this combined notice. Evaluation: Award will be based on Low Price Technically Acceptable quote received in response to this combined notice for determining which offer is of best value to the Government for meeting or exceeding the requirements of the statement of work as stated herein. Qualified offers are required to be licensed in the State of Hawaii for providing Trash/Garbage Collection and Disposal services on the Island of Oahu, Hawaii. Technical Acceptability: Currently operating as a trash collection company on the island of Oahu, Hawaii, for collecting trash and refuse and commercially licensed by the State of Hawaii for disposal of such refuse in a State designated landfill site. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-06, dated 09-10-2019. The associated North American Industrial Classification System (NAICS) code for this procurement is 562111, with a small business size standard of $41.5M. The services required are covered under the Service Contract Act (SCA) wage determinations for services on Oahu, Hawaii for a one-year base period and four one-year option periods. The applicable SCA wage determination is attached. Evaluation of Options: Except when it is determined in accordance with FAR 17.206(b) not to be in the Government s best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Evaluation of options will not obligate the Government to exercise the option(s). Contractor shall furnish all labor and equipment to provide trash/refuse removal for the Department of Veterans Affairs, Center for the Aging (building 110), E-wing building, and Ambulatory Care Center (ACC, building 30), 459 Patterson Road, Honolulu, Hawaii 96819-1522, on the grounds of Tripler Army Medical Center, Honolulu, Hawaii, and the Department of Veterans Affairs, Warehouse located at 3375 Koapaka St Honolulu HI 96819. A.2. Contractor will ensure the containers are rust free, neatly painted, labeled as required and maintains a presentable appearance at all times. Containers shall be in a clean and deodorized condition at all times. Rental containers shall be steam cleaned once a month. Cleaning shall not be performed on Government (VA) property; and must be performed prior to returning the container to the VA after disposal. The contractor at no additional cost to the Government shall replace containers that are considered unacceptable by the Contracting Officer s Representative (COR). A.3. Contractor shall ensure they are in full compliance with all state and local codes when disposing of all refuse. If refuse is dumped or spilled on the ground during pick-up, the contractor will be responsible to pick-up and clean up at contractor s expense. A.4. Contractor will provide a 24-hour emergency number. Emergency service shall be provided within two (2) hours of notification. A.5. Safety Requirements: Contractor will ensure all safety requirements are followed when servicing the containers. Any safety related discrepancies or hazards noted by the contractor when servicing should be corrected immediately or reported to the VA for correction. A.6. Center for the Aging (Building 110): Contractor is to provide two (2) six cubic yard containers with automatic locking lids at building 110 loading dock at all times. Containers need to be moved from the loading dock with a stinger/fork truck when emptying. Contractor will be required to empty the containers three (3) times a week on Tuesdays, Thursdays, and Saturdays. The containers must be emptied only between the hours of 10am and 3pm. A.7. E-Wing Building: Contractor is to provide one (1) eight cubic yard container with automatic locking lid at the back of the building s loading dock at all times. Contractor will be required to empty the container three (3) times a week on Tuesdays, Thursdays, and Saturdays. The container must be emptied only between the hours of 10am and 3pm. A.8. ACC (Building 30): Contractor is to provide two (2) eight cubic yard containers with automatic locking lids at building 30 loading dock at all times. Contractor will be required to empty the containers three (3) times a week on Tuesdays, Thursdays, and Saturdays. The containers must be emptied only between the hours of 10am and 3pm. A.9. VA Warehouse: Contractor is to provide one (1) four cubic yard container with automatic locking lids at 3375 Koapaka St Honolulu, HI 96819. Contractor will be required to empty the containers two (2) times a week on Tuesdays and Fridays. The containers must be emptied only between the hours of 10am and 3pm. A.10. The Contracting Officer is responsible for the administration of this contract. The Contracting officer is the only individual authorized, to the extent indicated in this contract, to take action on behalf of the Government, which may result in changes in the contract terms, to include deviations from the statement of work, details, and performance schedules. A.11. Communications pertaining to contractual matters shall be addressed to the Contracting Officer. No changes in or deviation from the scope of work shall be affected without a written modification to the contract executed by the Contracting Officer. No oral statements of any person whosoever will, in any matter or degree, modify or otherwise affect the terms of this contract. A.14. FEDERAL HOLIDAYS: VA observed Federal Holidays are: New Year s Day Martin Luther King Jr. Day Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day If a performance day falls on a Federal holiday, performance may be rescheduled with the appropriate COR. A.15. DESIGNATION OF CONTRACTING OFFICER S REPRESENTATIVE (COR): A Representative of the contracting Officer (COR) will be designated to represent the Contracting Officer in furnishing technical guidance and advise under this contract. The foregoing is not to be construed as authorization to interpret or furnish advice and information to Contractor relative to the financial or legal aspects of the contract. Those matters are the responsibility of the Contracting Officer and shall not be delegated. A.16. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: Upon contract award, all contract personnel shall be subject to the appropriate type of background investigation or screening per VA/VHA policy as delineated below and must receive a favorable adjudication from the local VA facility or VA Security and Investigations Center (SIC) depending on the type of investigation/screening required. This requirement is also applicable to all subcontract personnel. If the investigation or screening is not completed prior to the start date of the contract, the Contractor will be responsible for the actions of those individuals they provide to perform work for VA. Contract personnel who previously received a favorable adjudication as a result of a Government background investigation or screening may be exempt from this contract requirement provided that they can provide documentation to support the previous adjudication. Proof of previous adjudication must be submitted by the Contractor to VA Contracting Officer. Proof of previous adjudication is subject to verification. Some positions maybe subject to periodic re-investigation/screening. A.16.1. Position Risk/Sensitivity For all positions required under this contract, the position risk/sensitivity has been designated as: Low Risk/Non-Sensitive A.16.2. Background Investigation/Screening Since it is anticipated that the Contractor or contract personnel will be providing services at a VA facility(s) for more than 180 days under a single contract or series of contracts, the background investigation/screening commensurate with the requirements of this contract is: Background Investigation - National Agency Check with Written Inquiries (NACI) A.16.3. Contractor Responsibilities- The Contractor shall prescreen all personnel to ensure they are able to read, write, speak, and understand the English language. The Contractor shall submit or have their contract personnel submit the following required forms to the VA Contracting Officer, or designee, within five (5) business days of contract award (electronic forms will be made available by the contracting officer; the VA Contracting Officer will arrange a time for contract personnel to complete item iv.): NACI Requirements Standard Form 85, Questionnaire for Non-Sensitive Positions Standard Form 86A, Continuation Sheet for Questionnaires Optional Form 306, Declaration for Federal Employment FD 258, U.S. Department of Justice Fingerprint Applicant Chart and/or Electronic Fingerprint Verification Prior to completing the required forms, contract personnel should review Common Errors on Security Questionnaires, Example_SF85, Example_OF306, and General Questions and Answers about OPM background Investigations in order to ensure the forms are properly completed. Once the items requested in paragraph 3. b. above are received, VA will pre-screen these items for completeness, and forward them to the appropriate party(s) in order to initiate the required background investigation(s) or screening(s) within fourteen (14) calendar days of appointment. Only after the VA Contracting Officer notifies the Contractor that the background investigation(s) or screening(s) was initiated shall the Contractor be authorized to provide services under the contract. As previously stated, if the investigation or screening is not completed prior to the start date of the contract, the Contractor will be responsible for the actions of those individuals they provide to perform work for VA. The Contractor, when notified of an unfavorable determination by the Government, shall withdraw the contract person from consideration of working under the contract. Failure to comply with these Contractor personnel security requirements may result in termination of the contract for default. A.16.4. Government Responsibilities- The VA Contracting Officer will arrange a time for contract personnel to complete the fingerprint portion of this requirement, if any, and the Contracting Officer s Representative will responsible for performing any duties assigned by the VA Contracting Officer with regard to fulfilling the Contractor personnel security requirements described herein. Upon receipt, the local VA facility or VA SIC, depending on the type of investigation/screening required, will review the accuracy of the items requested in paragraph D.19.3. b. above, and forward these items to OPM to conduct their portion of the background investigation or screening, as applicable. The VA facility will pay for any portion of the investigation or screening conducted by OPM in advance. Depending on the type of investigation/screening required, the local VA facility or VA SIC will notify the VA Contracting Officer after adjudicating the results of the background investigation or screening. e. The VA Contracting Officer will ensure that the required investigations or screening have been completed or are in the process of being requested.
