THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) is soliciting businesses to provide snow removal and de-icing services at the Tomah VA Medical Centers Wausau CBOC (Community Based Outpatient Clinic). The Contractor shall furnish all labor, material, equipment, transportation and supervision necessary for snow removal and de-icing services. The following NAICS Code and Small Business Size Standard has been identified for this requirement: 561790 Other Services to Buildings and Dwellings with a Size Standard of $7.5 Million in annual revenue. Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. Capability statements shall also include: DUNS number Organization Name and Assumed Name/dba (if applicable) Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size (small business or other than small business) Socio-economic status (If SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Organization experience in performing these types of services Copies of applicable certifications and/or accreditations (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 561790. If another NAICS code may be applicable, please identify the NAICS in your capability statement along with your corresponding socio-economic status. Therefore, we recommend that you monitor the FBO website for all information relevant to desired acquisitions. ***** End Word Document - 'FBO-DESCRIPTION: FBO Posting -VISN 12 - Education' ***** STATEMENT OF WORK (SOW) Wausau CBOC Winter Surface Maintenance Contract Title: Wausau CBOC Winter Surface Maintenance Background: Currently, the Tomah VA Medical Centers Wausau CBOC has extended the existing building/facility lease. The extended lease was negotiated and will not have ongoing snow removal for the parking lot, sidewalks and handicapped ramp. To maintain patient and staff safety during inclement winter conditions snow or ice removal is required. The following SOW details the requirements that are needed for the Tomah VA Medical Center to provide for all required snow removal of their Wausau CBOC Clinic. Scope: Provide all labor, material, equipment, transportation and supervision necessary for snow removal and de-icing services at the Tomah VA Medical Centers Wausau CBOC (Community Based Outpatient Clinic). Snow removal and de-icing applications shall be provided on the following Wausau CBOC surfaces; Parking lot with approximately 55 parking spaces All sidewalks and handicapped ramps. All clinic entrance/exit stairs and entryways. Service access for Waste Management companies to clinic dumpster. Six (6) foot by six (6) foot access around fire hydrant(s) that serve Wausau CBOC. Services shall be performed in a manner that will maintain a satisfactory and safe environment for patients, families, staff, and visitors during periods of snow and ice. Contractor shall provide snow removal equipment to include, but not limited to, Contractor-owned trucks, plows, loaders, and de-icing spreaders. Contracted services are estimated to be required from approximately November 1 through May 1 for each period. Contractor shall provide a phone number and contact name of a Contractor employee(s) who shall be available on a 24/7 basis. All the parking lot space shall have de-icing salt/sand mix material applications after required snow removal is complete. All sidewalks, handicapped ramps, exterior stairs and clinic entryways shall have salt applications after required snow removal is complete. Snow removal shall take place anytime there is new snow accumulation of between 1 to 1.5 inches of snow, at a minimum. In the case of overnight/evening or early morning snow or ice storm events, for Clinic patient and staff safety, the Contractor shall have snow removal and de-icing material in the form of salt-salt/sand mix applications, as stated in above Scope of Work, completed prior to 6:30 am on any given business day. When daytime or overnight snowfall is greater than (6) inches, with blizzard or extreme ice conditions, the contractor shall clear all areas to the best capability of his or her labor and equipment. Snow and ice shall be removed from and around all fire hydrants. Contractor shall provide a cleared area six (6) foot by six (6) foot around said fire hydrants that serve the Tomah VA Medical Center s Wausau CBOC. No snow should be plowed or placed on any landscaping shubbery. Contractors shall be responsible for any damage to concrete curbs, sidewalks, ramps or other Wausau CBOC parking or sidewalk appurtenances, when incurred during the performance of this contract. The Contractor shall immediately remove ice to provide secure footing or safe driving conditions by the snow removal operation. All salt for the areas for which the Contractor is responsible for shall be provided by the Contractor under this agreement. The successful Contractor shall be liable to ensure that adequate source of salt and sand/salt mix is readily available throughout the duration of the contract. Under no circumstances shall the contract price be increased for the Contractor s failure to obtain salt necessary to perform services. Contractor shall be completely responsible for removing the snow and its disposal off the Wausau CBOC premises if agreed upon stockpile locations/areas exceed capacity.Snow removal shall be piled on grass or areas, which will be identified with the successful contractor and the COR following contract award. Service Hours and Federal Holidays: Normal hours of coverage are Monday through Friday from 6:30 am to 4:30 pm, excluding holidays. Federal Holidays observed by the VAMC are: New Years Day Labor Day Martin Luther King Day Columbus Day Presidents Day Veterans Day Memorial Day Thanksgiving Day Independence Day Christmas Day General Requirements: The contractor shall obtain all other necessary licenses and/or permits required to perform all work under this contract. Contractor is responsible to replace any items damaged by the contractor, sub-contractor, their employees or equipment at no extra charge to the Government. Contractor personnel must conduct their work in a manner that does not interfere with the normal functions of the facility and will stop work if asked by the CO/COR as required if interruptions do occur. Risk Control The Contractor shall take all necessary precautions to protect government property and shall repair any damage caused by their efforts. All work affecting patients shall be coordinated in advance by the Government and the Contractor to have minimal impact on the patients. Place of Performance.
