- Solicitation amendment 0001. **Solicitation Amendment 0001 - Is issued to extend proposal submission due date to September 11, 2019 at 17:00 EST. Additionally, one question is answered: Question: "Can we expect any utilities to be found in the area where the fall protection system installation/repair will take place?" Response: Yes. There is a possibility to have it near the working area. Please read GENERAL NOTE No. 1 on the drawing cover sheet (Drawing No. 3-1693, sheet 1 of 4) for detailed information. It shall be the contractor's sole responsibility to field verify the extent of work, working conditions and constraints prior to bidding. **End of Solicitation Amendment 0001 text.** Item Description: Maintenance and Repair to Tie-Back Anchors, Buildings 20 and 21, DSCC Columbus Ohio Scope of Work: Maintenance and Repair to Tie-Back Anchors, Buildings 20 & 21 at Defense Supply Center Columbus in Columbus, Ohio. Destination Information: DLA Land and Maritime, Columbus OH Period of Performance: 180 days from notice to proceed The magnitude of this construction project in terms of estimated cost is between $500,000 and $1,000,000 The award will be a firm fixed price contract. Award will be based on the lowest evaluated price of the technically acceptable proposal meeting all of the solicitation requirements from responsible offerors with acceptable past performance. There will be one award made for the solicitation. Award may be made without discussions. The following summary is for the purpose of general information and is not intended to include or describe every feature or item or to define the scope of work. It is expected that prospective contractor will attend the site visit and thoroughly familiarize themselves with the project prior to submitting a proposal. In addition, the complete solicitation, statement of work, and drawings will be available on the solicitation issuance date. The Contractor shall furnish all labor, material, supplies, equipment, tools, delivery, disposal and supervision required in performing all operations in conjunction with this project complete, in strict accordance with these specifications for Project CSC-19003 and Contract Drawing No. 3-1693 subject to the terms and provisions of this contract. MANDATORY CONTRACTOR QUALIFICATIONS: To be Technically Acceptable, qualified offerors must provide information regarding the technical qualifications of the proposed project staff, to include the individuals who will be responsible for the following functions: Written qualification statement for the individual who will be responsible for Project Management of the project. Qualifications: The Project Manager must have a minimum of 5 years' experience as a Project Manager or Superintendent on projects of similar size and complexity. Written qualification statement for the individual who will be responsible for the design work for the structural members to support tie-back anchors & proof load testing, inspections and re-certifying of the tie-back anchor systems of the project. § Qualifications: The individual shall be one of the licensed Structural Professional Structural Engineer and must have a minimum of 5 years' experience as a registered structural engineer and performed structural framing design work and work as proof load testing engineer for similar projects of similar size and complexity. § The individual shall be one of the certified member contractors of the International Window Cleaning Association (IWCA) and shall have a minimum of 5 years' experience as a proof load testing engineer in accordance with the regulations of OSHA, ANSI & IWCA 1-14.1 for similar projects of similar size and complexity. Written qualifications statement for the individual who will be responsible for Quality Control in accordance with General Requirements Section 01 45 00.10 Paragraphs 1.6.1. Qualifications: An individual with a minimum of 5 years combined experience as a superintendent, inspector, QC Manager, project manager, or construction manager on similar size and type construction contracts which included the major trades that are part of this contract. The individual must be familiar with the requirements of the EM 385-1-1 and have experience in the areas of hazard identification and safety compliance. Construction Quality Management Training In addition to the above experience and education requirements, the QC Manager shall have completed the course Construction Quality Management for Contractors and will have a current certificate. Written qualifications statement for the individual who will be responsible for safety in accordance with FAR 52.236-13 Accident Prevention (Nov 1991). Accident Prevention includes compliance with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910; and compliance with all pertinent provisions of the latest version of U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, in effect on the date of the solicitation. This individual will be designated as the Site Safety and Health Officer (SSHO). Qualifications and Training The SSHO(s), as a minimum, must have completed the 30-hour OSHA Construction safety class or as an equivalent, 30 hours of formal construction safety and health training covering the subjects of the OSHA 30-hour course (see Appendix A, paragraph 4.b) applicable to the work to be performed and given by qualified instructors. > The SSHO is also required to have five (5) years of construction industry safety experience or three (3) years if he possesses a Certified Safety Professional (CSP) or safety and health degree. The written qualifications statement shall be certified and signed by a principle of the offeror. Offerors shall submit a resume of each key individual. This will be evaluated to determine if they have the appropriate technical training, certifications, and previous construction experience to meet the technical requirements of a project of this nature. If any of the submitted individuals will perform more than one of the listed functions, this is acceptable as long as the submitted documentation identifies any and all functions the given individual will be performing. Certain skilled, experienced, professional and/or technical personnel are essential for successful accomplishment of the work to be performed under this contract. These are defined as " key personnel " and are those persons whose resumes are submitted as part of the technical/business proposal for evaluation. The contractor shall use key personnel as identified in its proposal during the performance of this contract and will request contracting officer approval prior to any changes. Requests for approval of any changes shall be in writing with a detailed explanation of the circumstances necessitating the change. The request must contain a complete resume for the new key personnel and any other pertinent information, such as degrees, certifications, and work history. New key personnel must have qualifications that are equal to or higher than those being replaced. The contracting officer will evaluate the request and notify the contractor whether the requested change is acceptable to the Government. Written qualifications statement for the individual who will be responsible as the authorized TPO Roofer of the Firestone Building Products, Inc. The remaining two factors to determine technical acceptability are past performance and technical construction capability. PERIOD OF PERFORMANCE: All work associated with the performance of this project must be completed within 180 calendar days after receipt of the Notice to Proceed. The solicitation will close at 10:00 am EST (Eastern Standard Time) on or about 9 September 2019. SITE VISIT: A site visit for this project is tentatively scheduled for Tuesday, 20 August 2019 at 9:30 am at Building 308 (verify that there has been no date change in the solicitation document). Access to DLA Land and M aritime, Columbus, Ohio is restricted. All individuals wishing to attend the site visit will be required to request access no later than 24 hours prior to the site visit. Those individuals wishing to attend must provide their name and corporate affiliation to the Contract Specialist by e-mail at Scott.Wesseler@dla.mil. Individuals will need to enter the Center through the Visitor Processing Center and a valid federal or state issued photo identification will be required.
