This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-04, dated 09-06-2019. The associated North American Industrial Classification System (NAICS) code for this procurement is 811219, with a small business size standard of $22.0M. The Department of Veteran Affairs Pacific Islands Health Care Services (VAPIHCS) Honolulu is seeking to negotiate a sole source award to the Hawai i Health Information Exchange for on-line subscription services and training within the state of Hawaii. Service requirement is stated for the following: Services Line Item Description Quantity Unit Price Total Price 0001 Medtronic Paceart Optima Annual License renewal and technical support in accordance with the Statement of Work 1 EA 0002 On-site training for Medtronic Paceart Optima 1.8 database. 1 EA STATEMENT OF WORK PACEART Customer Database Service Medtronic USA, Inc. ( Paceart ) The contractor shall provide all labor and personnel necessary to provide software customization and data query support as defined in this Statement of Work (SOW). 1. WORK TO BE PERFORMED: One year license agreement for the use of the PACEART system software. Remote application support Monday to Friday 7AM to 5PM CST. Review VA Portal application s interaction with the VA National Cardiac Device Surveillance Center s Paceart Optima Database. Review VA Portal stored procedures that query the Optima Database and recommend optimizations of the queries to improve performance and minimize interference with the Optima application. Review and document changes that the VA National Cardiac Device Surveillance Center has made to their Optima database to assess any negative impact to the Optima application. Evaluate performance of VA Portal and Optima Database for possible inclusion in a future release of the Optima product. The result of which may be used for future design in the Paceart Software. Onsite training with two technicians for two days in both the Washington DC and San Francisco Office on Paceart Optima 1.8 once installed. This agreement does not include the following: Review or creation of non-portal or custom queries, Database configuration analysis or production implementation of recommended changes. 2. SPECIAL WORK REQUIREMENTS: Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. 3.
