- TUSCALOOSA VA Medical Center NCO 07 Contracting 3701 Loop Road East Tuscaloosa, AL 35404 September 5, 2019 Subject: Solicitation 36C24719Q1072, Gutter and Down Spout Cleaning: Tuscaloosa VA Medical Center- Request for Quote Dear Prospective Offerors: The VA Medical Center invites your firm to submit a quote for the subject requirement as indicated in the Price/Cost Schedule of the solicitation. The Government intends to make an award of a firm-fixed price contract for this procurement. Please submit a quote in accordance with the following instructions: Complete blocks 12, 17a, 30(a) (b) and (c) of the attached SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the Solicitation. The Solicitation constitutes the model purchase contract. Complete Contractor information on page 3 of the solicitation Complete PRICE/COST SCHEDULE starting on page 6 of the solicitation Submit quotes on company letterhead also. Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation. Your quote shall be signed by an official authorized to bind the firm and be submitted via email on or before 10:00 AM CST on Wednesday, September 11, 2019. All communications prior to award concerning the solicitation shall be directed to LaShondra Wynn at lashondra.wynn@va.gov.
