- PD In Pateinet Peds RN Provision and Clauses Performance Work Statement In Patient Peds RN This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. This Request for Quote (RFQ) is for one (1-5) Registered Nurses - In Patient Pediatrics on an "As Needed Basis" to provide uninterrupted direct patient care services to be performed in the In Patient Pediatrics Department at Gallup Indian Medical Center (GIMC) located in Gallup, New Mexico. Hours are only estimated hours, the Government does not guarantee the contractor to work the full number of hours identified in the Performance Work Statement. Registered Nursing Services for the specified department shall be performed in accordance with the attached Performance Work Statement (PWS). Period of Performance: September 30, 2019 through December 31, 2019 as the Base Period with an option to extend beginning January 1, 2020 through December 31, 2020. The assignment as Registered Nursing Services for the department are to be performed for the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours are only estimated. Review the Statement of Work for full details of tour of duty and special requirements. CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. As a Physician, contractor shall provide prescribed medical treatment and personal care services to American Indian and Alaska Native beneficiaries. Duties include documenting observations, assessments, and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of competency standards. Our facility is seeking individuals who possess good organizational skills which enable completion of work, a demonstrated ability to assume leadership in a patient centered program, and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. Background Investigation: Selected qualified candidate will undergo FINGERPRINTING and Security Clearance processes before commencement of their tour of duty. Fingerprint Card: FBI FD-258 may be provided by the GIMC Human Resource Department: Fingerprinting MUST be cleared and approved prior to contract assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for Pediatric Physician services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant sub-factors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the position and the PWS to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Factor 1: Agency's Past Performance Sub-factor: Number of similar contracts (Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation). Factor 2: RN Nursing Qualification Requirements MUST BE PROVIDED. A complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information, Professional Skills and all relative documents for proposed candidate. Refer to the Performance Work Statement Sections 5.0 & 6.0 for a listing of requirements. Medical Evaluation: The contractor shall possess a health status allowing physical and mental ability to carry out required functions. This is to be verified on an annual basis by the employing agency or a copy of a physical examination provided by the contractor/practitioner. Immunization: Contractor shall provide evidence of current immunization documentation for MMR (measles, Mumps, and Rubella) Diphtheria and Tetanus and DT Booster, OOD status, or as required by the Navajo Area IHS immunization policy. A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Factor 3: Candidate Current / Valid Contact Information Subfactor: Mailing Address, Phone Number & Email Address Subfactor: Provide Three (3) current references with active telephone numbers for candidate. Factor 4: Registered Nursing Services Availability Subfactor: Upon fingerprinting clearance and credentialing clearance, the RN shall be available as soon as possible to start. Subfactor: The RN shall be flexible to allow for short term (13 weeks) or a long term (1 year), where feasible, include intermittent support plans to meet unplanned needs that develop on a frequent basis. Such as if RN needs to take a lengthy time away from contracting responsibilities then who will be the replacement. PROPOSAL SUBMISSION INSTRUCTIONS: 1. Quotation. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate that includes travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). 2. Past Performance 3.
