Chapter VI: Other Than Full and Open Competition (OFOC) SOP Attachment 4: Single Source Justification for SAP under $150K OFOC SOP Revision 05 Page 1 of 1 Original Date: 03/22/11 Revision 05 Date: 05/02/2016 JUSTIFICATION FOR SINGLE SOURCE AWARDS IAW FAR 13.106-1 (OVER MICRO-PURCHASE THRESHOLD($3.5K) BUT NOT EXCEEDING THE SAT ($150K)) IAW FAR13.104, COs must promote competition to the maximum extent practicable to obtain supplies and services from the source whose offer is the most advantageous to the Government, considering the administrative cost of the purchase. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). IAW FAR13.106-3(b)(3),COs are required to include additional statements explaining the absence of competition (see 13.106-1 for brand name purchases) if only one source is solicited and the acquisition does not exceed the simplified acquisition threshold (does not apply to an acquisition of utility services available from only one source) or supporting the award decision if other than price-related factors were considered in selecting the supplier. This template when completed can be used to document single source awards IAW FAR13.106-3(b)(3). Note: Statements such as "only known source" or "only source which can meet the required delivery date" are inadequate to support a sole source purchase. ACQUISITION PLAN ACTION ID: 626-19-3-313-0152 1A. PROJECT/TASK No. 1B. ESTIMATED AMOUNT: $128,947.67 2. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: The TVHS VA Medical Center needs IOA Healthcare specialty Sleeper Exam Chair Recliners. The IOA exam chair recliners will allow patient care and staff efficiency and safety in the hospital. The Sleeper Exam Chair Recliners are the only recliners with Trendelenburg, powered hydraulic vertical lift, reverse recline and all automatic wheel locking continuous footrest, and offers long term sitting comfort. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE SOURCE, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: IAW FAR13.106-1(b), Contracting Officers solicit from a single source for purchases not exceeding the simplified acquisition threshold. Contracting Officers may solicit from one source if the Contracting Officer determines that the circumstances of the order action deem only once source reasonably available. After an evaluation of various exam chair recliners, contracting officer have determined that there are no other vendors that can provide chairs with the exact specification that IOA can provide that will meet the hospital s requirement. The IOA exam chair recliner has a hydraulic lift that meets ADA 18.5 seat height. The chair is able to recline in any position. Every feature adds to patient and staff comfort and safety. The gliding motion feature provides stimulation for patients reducing the need to wander. The auto lock feature prevents patients from falling. After comparing various recliners and adaptive chairs and positioning seating devices there are no other sleeper recliners comparable. 4. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: Market research was conducted: VetBiz, SBA, GSA, FBO, and IFCAP, and previous purchase orders. Various exam chair recliners have been analyzed for a several month period by the clinicians and subject matter experts in the hospital: CHAMPION 85/59 Series: 300# weight limit, no Trendelenburg, does not elevate, no central lock CHAMPION Continuum Series: 400# weight limit, no Trendelenburg, does not elevate, no central lock WINCO 5670 Series 450# weight limit, No central lock, does not elevate. The Stryker does not have all of the features needed and is not meant for long term sitting. Just Medical Omega 330# weight limit, no Trendelenburg, does not elevate, no central lock. Broda 250-600# weight limit, does not elevate, no central lock. Due to not being able to find a Sleeper Recliner exam chair comparable to the IOA we need to publish an intent to sole source. Our SBS (Small Business Specialist has approved this JFOC. 5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
