- 508 Evaluation Criteria Statement of Work This is a Small Business Set Aside Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a fixed price purchase order for Financial Data Services on a sole source basis to Davis and Associates, 5920 Great Start Drive, Suite 20, Clarksville, MD 21029-1357. The anticipated purchase order is for one Base Year and 4 Option Years. The Contractor shall provide the services in accordance with the attached Statement of Work and Technical Evaluation Criteria. 'All business proposals final price shall be submitted in whole dollars only, rounded to the nearest dollar.' The North American Industry Classification Code (NAICS) is 541990 All Other Professional, Scientific, and Technical Services. The Request for Quote (RFQ) is being set aside for small business concerns. Responses shall be no more than fifteen pages and shall include the following: 1) Contractor's Plan to perform the Statement of Work dated July 31, 2019; 2) Fully loaded labor rates and anticipated hours. All responses will be evaluated by utilizing the Lowest Price Technically Acceptable (LPTA) Source Selection Process. All responses must be sent electronically in MS Word format to renee.wallaceabney@cms.hhs.gov on September 9, 2019 by 2:00 pm EST. to Renee Wallace-Abney, All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated.
