- NOTICE OF INTENT. The Department of Veterans Affairs, South Central VA Health Care Network (VISN 16) intends to award a sole-source, firm fixed price BPA (Blanket Purchase Agreement) to Roche Diagnostics Corp., 9115 Hague Road, Indianapolis, IN 46250 in accordance with FAR Subpart 8.405-6. The BPA will be effective October 1, 2019 through September 30, 2020. This requirement is for Accu-check Inform II test strips, controls and linearity kits for use with Roche point of care meters. The BPA will provide for continued usage of equipment currently on-site at VISN 16 Medical Centers and Outpatient Clinics. VA personnel utilize the equipment and supplies to perform patient WBG (whole blood glucose) testing. The NAICS is 334516 with a small business size standard of 1,000 employees. This is for informational purposes only and is not be construed as a commitment by the Government. No solicitation is available. Any information submitted in response to this notice of intent is strictly voluntary.
