Description
THIS IS NOT A SOLICITATION. This is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The Veterans Health Administration (VHA) Regional Procurement Office, Network Contracting Office (NCO)-16 in Houston, Texas is currently conducting market research to locate qualified, experienced and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought (SS) announcement. The Department of Veterans Affairs (VA) is not, at this time, seeking quotes and will not accept unsolicited quotes. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this SS; all costs associated with responding to this SS will be solely at the interested vendor's expense. Any information submitted by respondents to this SS is strictly voluntary. All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set-asides for the above-noted service. This is NOT A REQUEST FOR QUOTE or an announcement of a solicitation. I. OVERVIEW: The Michael E. DeBakey Veteran Affairs Medical Center (MEDVAMC) has a requirement for Physics Surveys of Diagnostic imaging equipment as required by VA National Health Physics Program, Texas Regulations for Control of Radiation, and US Food and Drug Administration. NAICS: 541690 Other Scientific and Technical Consulting Services SIZE STANDARDS: $15M The work includes but is not limited to the following: Contractor shall furnish all labor, tools, phantoms, and equipment to perform annual inspections, tests, and calibrations according to the specifications listed in the performance work statement (PWS). All work shall be documented and completed according to current State of Texas, American Association of Physicists in Medicine, FDA, and American College of Radiology requirements/guidelines. Prior to the first visit, contractor shall submit a schedule and sample documentation sheets for the government point of contact approval. All work shall be conducted by or under the auspices of a qualified medical physicist (QMP). A Qualified medical physicist (QMP) is a person who is certified by the American Board of Radiology, American Board of Medical Physics, American Board of Health Physics, the Canadian College of Physicists in Medicine, or State Licensure. Performance Period: Contract period of performance is for a base one-year period beginning October 1, 2019 to September 30, 2020 with four 12-month option periods. Subject to availability of funds. II. VETERANS FIRST CONTRACTING PROGRAM: The Department of Veteran s Affairs is dedicated to the Veterans First Contracting Program and encourages any certified Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) capable of furnishing the required services to respond. In accordance with Veterans Affair Acquisition Regulation (VAAR) sections 819.7005 and 819.7006, if a sufficient number of eligible SDVOSB or VOSB firms are identified during market research the acquisition will be set-aside for SDVOSB or VOSB participation. III. INFORMATION REQUESTED FROM INDUSTRY: Please submit your response (capability statement) with no more than 5 pages in accordance with the following: Include the business name, physical address, point of contact: email address and phone number of the appropriate representative of your company; DUNS number, NAICS code Indicate whether your company, subcontractors, teaming partners, joint ventures have a federal Socio-Economic status, (e.g., Small Business, Service-Disabled Veteran Owned Small Business, Veteran Owned Small Business, Woman-Owned Small Business, Disadvantaged Small Business, and Hub Zone). If Service- Disabled or Veteran Owned Small Business, is your company and/or partners registered in VA s VetBiz repository? What services or products are sub-contracted? Please provide a breakdown of what your company sub-contracts. Please provide percentages of the work you will be providing as self-performed and sub-contracted. If the company holds a Federal Supply Schedule (FSS) Contract, list the GSA, GWAC, IDIQ, Contract Number and/or BPA number with relevant Special Item Numbers (SINS) applicable to this requirement. Has contractor provided similar services? (Contractor shall provide in capability statement a list of active contracts for commercial, federal, state, and local governments. List of contracts shall provide communication of the government requirement illustrating the capability for comparison).
