Description Subject: Request for Information (RFI/Sources Sought), for Consultation & Support Services - Quality Assurance Performance Improvement (QAPI) Infrastructure Program Design, for the Department of Veterans Affairs, Central Offices (VACO) Geriatrics and Extended Care (GEC) office. This source sought RFI is being published in accordance with Title 38 U.S.C. § 8127.The VA Central Office(VACO) Program Executive Office (PEO) Geriatrics and Extended Care is in need of Consultation & Support Services for Quality Assurance Performance Improvement (QAPI) Infrastructure Program Design. The response from this RFI may result in the issuance of a solicitation. This sources-sought encourages potential contractors to submit a non-binding statement of interest and documentation substantiating they have a product they feel meets the stated Minimum Technical Requirement (MTR s). Further, it is intended to assess industry s capability to support and develop an adequate solution to the Governments continuing need(s). The Strategic Acquisition Center (SAC) is requesting responses from qualified manufacture business concerns; the qualifying NAICS code for this effort is 623110 Nursing Care Facilities (Skilled Nursing Facilities). This sources-sought is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran-Owned Small Business (SDVOSB/VOSB), Hub Zone, 8(a), small disadvantaged, women-owned small business, or large business) relative to NAICS 623110. Responses to this notice will be used by the Government to make appropriate acquisition decisions. After review of the responses to this notice, a solicitation announcement may be published. Responses to this RFI or sources sought are not considered adequate responses to any corresponding solicitation announcement. All interested offerors will have to respond to any resulting solicitation announcement, in addition to responding to this RFI or sources sought. BRIGHT SPOT IMPLEMENTATION STATEMENT OF WORK Type of Order: Firm Fixed Price Period of Performance: One year from contract award plus two option years. Place of Performance: VA Central Office, up to 30 VA Community Living Centers (CLCs), and up to 4 separate government (Employee Education Service EES) workshop venues. It is anticipated that the Government may provide meeting space only. All other work will be performed at the contractor s facility. The VA CLCs will be identified from the population of 134 existing VA CLCs. Specific CLCs, workshop locations, and visit dates are to be decided and approved by the COR. 1.0 BACKGROUND AND PURPOSE The purpose of this effort is to build a quality assurance performance improvement (QAPI) infrastructure to support the VA Central Office of Geriatrics and Extended Care (VACO GEC) vision for system-wide improvement in staff engagement, Veteran experience, resident-centered care, and quality outcomes through the CLC s Ongoing National Center for Enhancing Resources and Training (CONCERT). CONCERT seeks to engage and support Community Living Center (CLC) staff in successfully implementing CLC-based high-involvement QAPI. In FY 17 GEC achieved a staged rollout of the Bright Spots Program across all VISNs, focusing primarily on nursing staff and nurse leaders, through a train-the-trainer approach. This project used the Leadership Development for Staff Engagement in Individualizing Care and High-involvement Quality Improvement Closest to the Resident methods and techniques. In FY 18-19 CONCERT was created to expand the scope of the CLC quality improvement effort. Having completed the Bright Spots Program national roll-out in FY17 and the CONCERT expansion in FY18-19 (which included Learning Intensives with 39 CLCs), and thus needing to maintain these gains; and given that 95 CLCs remain that need Learning Intensive training as well as many other integration/coordination and expansion needs, GEC seeks to improve quality and person-centeredness of CLC care by implementing a 12-month (plus one option year) plan to: maintain the CONCERT gains achieved through FY19, integrate and coordinate CONCERT with other existing VACO programs, and extend the reach of CONCERT services. The expected results are creation of high-functioning, relationship-based CLC staff teams and team processes to improve staff functioning and morale and resident-centered care focus and skill, thereby improving the Veteran experience. This work builds upon, supports, maintains, integrates, and extends the CONCERT work that was begun during FY17 and expanded and continued in FY18-19 by VACO GEC, and provides operational support for VACO GEC s CLC Program Office. SCOPE This contract is intended to provide consultation and support to VACO leaders and staff, CONCERT staff, and field-based CLC leaders and staff to achieve effective system-wide improvement in staff engagement, Veteran experience, resident-centered care, and quality outcomes. The vendor will provide services to maintain the CONCERT gains achieved through FY19, integrate and coordinate the CONCERT with other existing VACO programs, and extend the reach of CONCERT services. Maintenance services consist of ongoing CONCERT staff consultation (mentoring and advising team regarding strategic planning, maintenance material development, and the team s roles as site support personnel for the CLCs). Integration/Coordination services consist of ongoing guidance and support to VACO GEC s CLC Program Office regarding strategic planning and regarding integrating/coordinating the CONCERT program efforts with: CLC Compare ratings, CLC surveys, CLC QAPI, MDS and other CLC field requirements, VACO GEC CLC Program Office technical support site visits, and other CLC-related VACO programs outside VACO GEC CLC Program Office (e.g., those in Office of Mental Health and Office of Nursing). Extension services include: a) Supporting Rapid Cycle Quality Improvement Learning Intensives (LIs) to the remaining 95 CLCs which have not yet participated in CONCERT LIs and to CLCs that previously participated and need additional training (late adopters); b) developing and delivering a workshop for the VISN GEC Leads and VISN Quality Management Officers (QMOs); and c) the preparation, planning, and consulting activities necessary to achieve (a)-(b). The task shall be successfully performed in a way that ensures successful completion of the following six (6) contract objectives. Objective 1 (Maintenance Objective): provide ongoing guidance and support in maintaining the CONCERT activities that were set in motion FY17-FY19 through Vendor ongoing CONCERT team participation (mentoring and advising team regarding strategic planning, maintenance material development, and the team s roles as site support personnel for the CLCs). Objective 2 (Integration/Coordination Objective): provide ongoing guidance and support to VACO GEC CLC Program Office regarding integrating/coordinating the CONCERT program efforts with: CLC Compare ratings, CLC surveys, CLC QAPI, MDS and other CLC field requirements, VACO GEC CLC Program Office technical support site visits, and other CLC-related VACO programs outside VACO GEC CLC Program Office (e.g., those in Office of Mental Health and Office of Nursing). Objective Three (Integration/Coordination Objective): provide guidance and support to the CLC Program Office strategic planning. Objective Four (Extension Objective): development and delivery of CONCERT Learning Intensive training and CONCERT team guidance and support for the CONCERT Learning Intensive site support. Objective Five (Extension Objective): The vendor will plan, development and carry-out the delivery of a 2-day workshop for the QMOs and GEC leads. Objective Six (Extension Objective): consultation, development and delivery of CLC restorative nursing topics, Minimum Data Set (MDS) training, and Patient-Driven Payment Model (PDPM) and impact of nurse staffing model. TASKS General Requirements The vendor will participate in meetings with CONCERT team members, VACO leaders and staff, and field-based CLC leaders and staff, which may also include attendance and participation at standing meetings and group and individual discussions. The vendor will complete LIs for up to 95 CLC who have not yet received LI instruction and LI instruction to up to 15 CLCs who require additional training ( late adopters"). The vendor will complete up to 4 face-to-face meetings per year with CONCERT team members and/or VACO GEC CLC Program leaders, and up to 40 CLC site visits total which will each include meetings with field-based CLC leaders and staff, touring and observing CLC areas, documenting observations and any best practices for inclusion/shaping the development of CONCERT materials, and completing 2 site visits each with the 15 CLCs participating in the late adopter LI. The vendor will provide a VISN GEC lead/QMO workshop. The vendor will provide consultation and training programs on CLC restorative nursing topics, Minimum Data Set (MDS) training, and Patient-Driven Payment Model (PDPM) and impact of nurse staffing model. The vendor is to provide training in a face to face for the new MDS changes that will be implemented Oct 1. They will also provide training via webinar for all 134 CLCs on restorative nursing program. The vendor will participate in planning/evaluation meetings with the CONCERT team members and VACO GEC CLC Program leaders regarding CONCERT planning, development, maintenance, integration, implementation, and evaluation. In all tasks, the Vendor is expected to use the Leadership Development for Staff Engagement in Individualizing Care and High-involvement Quality Improvement Closest to the Resident methods and techniques to provide the above services. Specific Mandatory Tasks and Associated Deliverables Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in this SOW. (Each task below is linked to the associated contract objectives detailed in section 2.0 above.) Task One (1)-Kick-Off Meeting [Objectives 1-6] Kick-Off Meeting five (5) business days from date of award (in person or via phone). The contractor shall hold a Project Kick-Off Meeting with the VA Project Manager (PM) and Contracting Officers Representative (COR). The contractor shall schedule the Kick-Off Meeting to be held within five (5) business days after contract award or as agreed upon between the VA PM/COR and contractor. 3.2.1.1 Deliverable: Kick-Off meeting five (5) business days from date of award and provide minutes to COR within (5) business days of kick-off meeting. TASK Two (2) Consultation and Materials Development Regarding CONCERT Maintenance and Integration/Coordination [Objectives 1-2] Task includes the following effort: Vendor Meetings with CONCERT team members, once per month, regarding Maintenance Efforts: Ongoing CONCERT team meeting participation (mentoring and advising team regarding strategic planning, maintenance material development, and the team s roles as site support personnel for the CLCs based on the Leadership Development for Staff Engagement in Individualizing Care and High-involvement Quality Improvement Closest to the Resident methods and techniques) Vendor Meetings with GEC Central Office leaders (and CONCERT team members as applicable) regarding Integration/Coordination efforts: integrating/coordinating the CONCERT program efforts with: CLC Compare ratings, CLC surveys, CLC QAPI, MDS and other CLC field requirements, VACO GEC CLC Program Office technical support site visits, and other CLC-related VACO programs outside VACO GEC CLC Program Office (e.g., those in Office of Mental Health and Office of Nursing) based on the Leadership Development for Staff Engagement in Individualizing Care and High-involvement Quality Improvement Closest to the Resident methods and techniques. Vendor will produce, contribute to, and/or review CONCERT materials (e.g., handouts and PowerPoint slides on CONCERT practices such as Watchlist Huddles, blog posts for VA Pulse site, guides for responding to survey and CLC Compare results using CONCERT principles [i.e., based on Leadership Development for Staff Engagement in Individualizing Care and High-involvement Quality Improvement Closest to the Resident methods and techniques], etc.) and other pertinent materials used for the purpose of training, informing, and guiding CONCERT maintenance. Vendor will advise and coach the CONCERT team members on all aspects of CONCERT maintenance including how to handle problems or issues that may arise; this will involve regular phone meetings with CONCERT team leads, sometimes up to multiple times per week Vendor will aid in the interpretation and dissemination of data analysis results related to CONCERT maintenance and integration efforts. Vendor will lead up to 5 two-day site visits per year to identify promising/best practices in the CLC field and use them to inform development of CONCERT dissemination materials, to inform CONCERT strategic planning and VACO GEC CLC Program integration/coordination, and to mentor CONCERT staff in effective site visit methods. Promising practice site visits will include meetings with field-based CLC leaders and staff, touring and observing CLC areas, and documenting for development of CONCERT materials. The vendor will provide just-in-time teaching to participating CONCERT team members and field CLC leaders/staff during site visits; The vendor shall attend and participate in CLC site standing meetings such as staff change-of-shift meetings, interdisciplinary team (IDT) meetings, quality improvement team (QIT) meetings, and nurse manager meetings. The vendor will meet with CONCERT team members before, during, and after site visits to provide mentoring, debriefing, feedback, and guidance on site visit planning, conduction, and follow-up skills including just-in-time teaching, interviewing, observation, documentation skills, and overall structuring and planning of visits. 3.2.2.1 (a) Deliverable: Each month the vendor shall submit a monthly report that lists each meeting attended and any recommendations/lessons learned. 3.2.2.1 (b) Deliverable: Promising Practice Site Visit reports (10 in total) capturing consultation effort, implementation recommendations, and lessons learned 14 days following each site visit. 3.2.2.1. (c) Deliverable: Promising Practice Site Visits at up to 5 CLCs per year (10 site visits in total). 3.2.2.1 (d) Deliverable: CONCERT maintenance and integration/coordination materials (e.g., handouts, PowerPoint slides, blog posts for VA Pulse site, best/promising practice materials, implementation guides, guides for responding to survey and CLC Compare results using CONCERT principles, etc). 3.2.3 TASK Three (3) CLC Program Office Strategic Planning [Objective 3] The vendor will participate in up to 8 face-to-face two-day meetings with VACO GEC CLC Program leaders and CONCERT team leaders to evaluate current status and resources needed to provide support to CLCs for improvement and formulate a CLC QAPI strategic plan. Vendor will accompany VACO GEC CLC Program leaders on one unannounced survey visit. 3.2.3.1 (a) Deliverable: Debriefing meeting with applicable CLC Program Office team members/CONCERT team leaders after unannounced survey visit. 3.2.3.1. (b) Deliverable: Up to eight (8) face-to-face two-day strategic planning meetings with VACO GEC CLC Program leaders and CLC Program staff and CONCERT team leaders/staff as applicable 3.2.3.1. (c) Deliverable: Report after each strategic planning meeting capturing consultation effort including recommendations and lessons learned 14 days after each meeting TASK Four (4) Community Living Center (CLC) Learning Intensives (LIs) [Objective 4] Task includes the following effort: LI for up to 95 CLCs who have not yet received LI instruction: Vendor will develop all materials for and deliver a series of 5 Learning Sessions and one action period consultation and training, as follows: Learning Session #1: up to 2 repetitions of a two-day face-to-face learning session, and a preparation call with VAMC leadership and VISN GEC leads Action Period #1 consultation and training: one individual virtual one-hour consultation for each participating CLC, and one half-day virtual training to be attended by all CLCs. Learning Session #2: up to 2 repetitions of a two-day virtual learning session Learning Session #3: one individual virtual one-hour consultation for each participating CLC, and two half-day virtual training sessions to be attended by all CLCs Learning Session #4: up to 2 repetitions of a two-day face-to-face learning session, and preparation call with VAMC leadership and VISN GEC leads Learning Session #5: up to 2 repetitions of a two-day face-to-face learning session LI for up to 15 CLCs who require remedial LI instruction ( late adopters ): Vendor will develop all materials for and deliver a series of 4 Learning Sessions, each for 2 days face-to-face, for a total of 8 days of training, and at least one and up to 2 preparation/coordination calls with VAMC leadership and VISN GEC leads The vendor will lead up to 2 site visits to each of the 15 CLCs The vendor will provide just-in-time teaching to participating CONCERT team members and field CLC leaders/staff during site visits; The vendor may also attend and participate in CLC site standing meetings such as staff change-of-shift meetings, IDT meetings, QIT meetings, and nurse manager meetings. Vendor will advise and coach the CONCERT team members on all aspects of the LIs, its application to resident-centered care initiatives, and how to handle problems or issues that may arise; this will involve regular phone meetings with project leads, sometimes up to multiple times per week The vendor will collect implementation information to develop the CONCERT IDT/QIT extension implementation materials; 3.2.4.1 (a) Deliverable: LI Learning Session reports (6 in total 5 learning sessions plus a report after the action period #1 consultation/training) capturing consultation effort, implementation recommendations, and lessons learned 14 days following each Learning Session. 3.2.4.1 (b) Deliverable: LI Face-to-Face Learning Sessions (8 total two-day learning sessions). 3.2.4.1 (c) Deliverable: LI virtual learning sessions (3 half-day sessions and 2 two-day learning sessions). 3.2.4.1 (d) Deliverable: LI virtual one-hour consultations (2 series of 95 one-hour consultations for a total of 190 one-hour consultations). 3.2.4.1 (e) Deliverable: LI preparatory informational call with VAMC leaders and VISN GEC leads (2 one-hour calls) 3.2.4.1 (f) Deliverable: Late Adopter LI Learning Session reports (4 in total) capturing consultation effort, implementation recommendations, and lessons learned 14 days following each Learning Session. 3.2.4.1 (g) Deliverable: Late Adopter LI Learning Sessions (4 two-day face-to-face learning sessions in total). 3.2.4.1 (h) Deliverable: Late Adopter LI preparatory informational call with VAMC leaders and VISN GEC leads (at least 1 and up to 2 one-hour calls) 3.2.4.1(i) Deliverable: Late Adopter LI Site Visit reports (30 in total) capturing consultation effort, implementation recommendations, and lessons learned 14 days following each site visit. 3.2.4.1 (j) Deliverable: Late Adopter LI Site Visits at up to fifteen (15) CLCs (30 site visits in total). 3.2.5 Task Five (5) GEC Leads/QMO Workshop (Objective 5) The Vendor will deliver a two-day workshop for the VISN GEC Leads and QMOs to train them on the CONCERT and IDT/QIT Extension. 3.2.5.1 (a) Deliverable: Report capturing consultation effort, implementation recommendations, and lessons learned 14 days following workshop. 3.2.5.1 (b) Deliverable: One (1) 2-day workshop for the VISN GEC Leads and QMOs. 3.2.6 Task Six (6) Nursing Training and Consultation (Objective 6) Vendor will develop all materials for and deliver a series of 3 two-hour virtual workshops on CLC restorative nursing topics. The vendor will repeat the workshop series up to three times. Vendor will provide consultation to VACO GEC CLC Program leaders about Minimum Data Set (MDS), Patient-Driven Payment Model (PDPM), and the impact on nurse staffing model. Vendor will develop all materials for and deliver a series of 3 two-hour virtual workshops on MDS, PDPM, and impact of nurse staffing model. The vendor will repeat the workshop series up to three times. 3.2.6.1. (a) Deliverable: Workshop reports (6 in total one per workshop series) capturing consultation effort, implementation recommendations, and lessons learned 14 days following each workshop Series. 3.2.6.1. (b) Deliverable: Virtual Workshops (up to 18 two-hour workshops total). MANAGEMENT REPORTING Submission The contractor shall submit to the Contracting Officer s Representative (COR) in draft form each deliverable for review and approval. The Government will complete their review of the draft deliverable within five business days from date of receipt. The contractor shall have five business days to deliver the final deliverable from date of receipt of the governments comments. An electronic copy of all final deliverables will be provided to the COR. The PM and/or designated representative will review and accept deliverables. Products found to be unacceptable or not meeting the intent of the task shall be redone by the contractor and considered to be within scope of this contract. Business Day is defined as Monday through Friday, excluding standard Federal Holidays and any other day specifically declared by the President of the United States to be a national holiday. 4.1 Monthly Reports Each month, the contractor shall submit an overview report which will contain the status of the acceptances of travel expenditures, changes, identify any implementation roadblocks, and present any risk mitigation strategies the contractor deems appropriate for the government to consider. Reports should be submitted no later than the second workday following the end of each calendar month. 5.0 DELIVERABLES SCHEDULE If for any reason, any deliverable cannot be delivered in the time schedule, the contractor shall provide a written explanation to the CO/ COR. This written transmittal shall include a firm commitment of when the work shall be completed. This notice to the CO/COR shall cite the reasons for the delay, and the impact on the overall project. The Contracting Officer will then review the facts and issue a response in accordance with applicable regulations. Minor changes to site visit start dates (plus or minus 7 days) may be made by the COR with 72 hours notice. Travel and per diem, if required, shall be reimbursed in accordance with VA/Federal travel regulations. It is anticipated the contractor will travel at least once to VA Central Office (or another agreed upon central location) and will travel for site visits as detailed in the previous sections. Each contractor invoice must include copies of all receipts that support the travel costs claimed in the invoice. Travel must be pre-approved by the COR. Travel must be priced separately in the price schedule. Local travel within a 50-mile radius from the contractor s facility is considered the cost of doing business and will not be reimbursed. This includes travel, subsistence, and associated labor charges for travel time. Travel performed for personal convenience and daily travel to and from work at the contractor s facility will not be reimbursed. Travel, subsistence, and associated labor charges for travel time for travel beyond a 50-mile radius of the contractor s facility are authorized on a case-by-case basis and must be pre-approved by the COR. 7.0 OTHER DIRECT COSTS (ODCs) ODCs, if required, will be reimbursed at contractor cost. Each contractor invoice must include copies of all receipts that support the ODCs claimed in the invoice and written substantiation for the incurred costs. 8.0 GOVERNMENT RESPONSIBILITIES VA shall provide information regarding the current Bright Spot initiatives. The Government shall provide meeting space for meetings that require the contractor to be present at VA offices.
