- STATEMENT OF WORK Scope: The contractor shall provide all labor, travel, materials and the necessary equipment to provide semi-annual and annual maintenance on Mitsubishi 1100B Uninterrupted Power Supply. Semi-annual and annual maintenance will include: physical inspections, electrical inspections, system operations check and follow through. All work shall be performed according to manufacturer s specifications and industry standards at Jesse Brown VAMC. Specific Tasks/Deliverables Task1: Semi-Annual maintenance will include: Visual Inspection: Inspect individual battery units for signs of electrolyte leakage, venting, overheating, terminal integrity, terminal corrosion, bulging, and general cleanliness Check battery room or area for proper temperature and ventilation. Cleaning: Battery cabinet interior/exterior free of dirt and debris. Battery units free of dust and debris. Data Recording/Measurements: Battery cabinet name plate information including date code. Ambient battery unit temperature at or near the highest physical location in the string. DC float voltage for each individual battery unit. Measure and record each battery units internal Ohmic values. Temperature of the negative terminal on each battery unit. Measure of DC ground faults. Battery Charger: Verify proper battery string float voltage and charging current per manufactures specifications. Measure and record all inter-cell and main terminal cable Ohmic values. Take apart and clean any connection that has an Ohmic value less than the manufacturer s recommended value or has a value that decreased by more than 20% below the baseline value. Any loose connections that are found will be torqued to the battery manufacturer s recommended value. Remedial Action: Develop remedial action plan with customer to replace battery units where the test values fall outside the battery manufactures specifications. Task 2: Annual maintenance will include: Physical Inspections: Internal/external visual inspections of UPS cabinet for signs of dirt and other foreign material. Visual inspection of internal components for signs of deterioration and or damage. Visual inspection of internal wiring signs of looseness deterioration and or damage. Visual inspection of environment for signs of impact too proper UPS operation. Electrical Inspection: Verification of proper software versions and all adjustable parameters. Verification of proper control power supply values. Verification of proper converter/rectifier operation. Verification of proper inverter operation Verification of proper cooling fan operation. Verification of proper static/internal maintenance bypass operation Verification that system voltages and currents are within specifications. Verification of proper display of system values. Review event/fault history menus System Operations Check: Verification of proper inverter forward and reverse manual transfer operation. Verification of proper inverter forward and reverse automatic (fault) transfer operation. Verification of proper external maintenance bypass operation. Verification of proper emergency (back up) mode of operation (after battery check). Verification of proper operation of peripheral products. Verification of proper operations of emergency power off (EPO) test. Verify UPS is supporting critical load and no alarms are present. Follow Through: Document and advise customer of all findings. Review equipment operation and event/fault history with customer. Make adjustments or take corrective action where possible. Develop a remedial action plan with customer to address major issues. Hours of Operation. Normal hours of coverage are Monday through Friday from 7:30 am to 4:30 pm CST, excluding holidays. All service/repairs will be performed during normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Place of Performance. Jesse Brown VA Medical Center 820 S. Damen Ave Chicago, IL 60612-3728 Period of Performance. Base Year: September 15, 2019 through August 31, 2020 Option Year 1: September 1, 2020 through August 31, 2021 Option Year 2: September 1, 2021 through August 31, 2022 Option Year 3: September 1, 2022 through August 31, 2023 Option Year 4: September 1, 2023 through August 31, 2024 Parts. Any and all parts will be paid separately outside this contract. Performance Monitoring. Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract. Direct Observation-(Can be performed periodically or through 100% surveillance.) Periodic Inspection-(Evaluates outcomes on a periodic basis. Inspections may be scheduled [Daily, Weekly, Monthly, Quarterly, or annually] or unscheduled, as required.) ACCEPTABLE PERFORMANCE The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action. UNACCEPTABLE PERFORMANCE When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file. When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's program manager. The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability. Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO. Security Requirements. Vendor will not have access to VA Sensitive Information, patient records, or data. For repairs or services performed during normal work hours, 7:30 a.m. to 4:30 p.m. at the Jesse Brown VA Medical Center, contractor s repairman shall report to the Contracting Officer Representative (COR). Contractors shall sign-in and obtain a badge and return the badge at the end of the day. After all work is complete, the contractor s repairman shall submit a written service report within 48 hours of the completed work outlining the services performed and completed to include location and parts used. Government-Furnished Equipment (GFE) / Government-Furnished Information. None. Other Pertinent Information or Special Considerations. All work shall be performed by personnel qualified to work on the specific equipment.
