Sources Sought Introduction The Tampa VA is seeking a requirement to provide a transitional care program that will generate patient s educational documents by utilizing existing information within VA systems. The solution will permit the use of the VA s Care Transitions Portal: The Veterans Information System and Technology Architecture (VistA) and Computerized Patient Record System (CPRS). The vendor shall implement an Integrated Care Transitions Portal user interface to create and print a Care Plan booklets for veterans as part of their discharged process. The Care Plan will include the following data to the extent that they are available within the system: · Branded Site Logo · Patient Aligned Care Team details · Patient Demographics · Medications · Appointment Information · Diet and Activity Instructions · Patient Education · Patient Care Instructions · My HealtheVet information B. Background The Tampa VA currently provides veterans with educational materials to help reduce preventable readmissions, improve compliance with medical plan of care, follow-up appointment attendance, and improve patient care transitions from acute to primary care settings. Scope: The vendor shall provide a solution for to the Tampa VA nursing administrative unit to support patients needs identified above. Maintenance and support services shall be included. Training: The contractor shall provide training as needed for any significant system performance changes. Virtual training will be preferred. The Tampa VA shall also request on-site training if warranted. Maintenance Services: The contractor shall provide day-to-day support (via e-mail and phone), software maintenance, and continual access to quarterly updates (new features, content, enhancements, and other updates to the software). Potential vendors must be aware of: Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.   No remanufactures or gray market items will be acceptable.   Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of contract or, solely at the VA s election, allowing the Vendor to  replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   Interested Vendors The applicable NAICS for this service is 541512.
