- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. (ii) Solicitation number 36C25719Q1171 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03. (iv) This solicitation is issued as unrestricted, full and open competition, with the associated NAICS code of 334516, which has a small business size standard of 1,000. (v) See attached schedule for the contract line item number(s), quantities, and options requested in this solicitation. (vi) This requirement is for providing a Molecular Cost-Per-Test (CPT) Program to the South Texas Veterans Health Care System, San Antonio, TX. See attached schedule for the location and the supplies and services that are to be provided. (vii) The anticipated base Period of Performance for the supplies is approximately 15 August 2019 thru 14 August 2020. The resultant contract will be considered a standing-order contract, where the supplies will be ordered on an as-needed basis throughout each period of performance in the contract. (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. See item xiii below for additional submission requirements. (ix) The provision at FAR 52.212-2, Evaluation-Commercial Items, is used in this solicitation. Evaluation of offers will be conducted as Lowest-Priced Technically Acceptable (LPTA). Offers with proposed equal equipment items will be evaluated to the specifications of the Cepheid equipment named herein and with the testing compatibility of the reagents listed in this solicitation. Offers that do not meet the equipment specifications and the reagents testing requirements will be assessed as Technically Unacceptable and will not be considered for award. Past Performance will be assessed by conducting a review and evaluation of the offeror s past performance records found in government performance systems. Offerors found with unsubstantiated unsatisfactory past performance records may be assessed as Technically Unacceptable. The lowest priced offer will be assessed first. If the lowest priced offer is assessed as Technically Acceptable, the offer will be considered the best value for the government and no other offers will be evaluated for Technical Capability. If the lowest priced offer is assessed as Technically Unacceptable, the next lowest priced offer will be assessed for Technical Capability, and so on, until an offer has been assessed as Technically Acceptable. It is possible that not all offers submitted in the solicitation will be assessed for Technical Capability. The offer of the lowest price, whose technical capability and past performance was found to be technically acceptable, will be considered the best value for the government. (x) Offerors whom have submitted an annual online representations and certifications (FAR provision 52.212-3) in the Systems and Award Management (SAM) do NOT need to return a completed copy of the provision with their offer. Offerors must be registered and have completed the provision at FAR 52.212-3 in SAM to be considered for an award from this solicitation. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Addenda to the included clause state that clauses that are incorporated by reference have the same effect as if they were given in full text. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. See attached schedule regarding the additional FAR clauses cited within the clause that are applicable to this acquisition. (xiii) This acquisition includes additional submission requirements. Other additional submission requirements are: (a). Solicitation, Schedule, Line Items. It is anticipated that not all parts of an offer can be edited into the provided solicitation document. Offerors may submit an offer using the attached SF1449 or submit an offer in their own format (PDF of Microsoft document compatible). Subline Item 0001AA, Equipment. This solicitation includes FAR provision 52.211-6, Brand Name, or Equal. Offers of equal items must provide the model number and the manufacturer name of the proposed equipment with the offer. Offers of equal items must include the specifications of the proposed instrument with the offer. Subline Item 0001AB, Equipment Maintenance Services. Offeror must provide a description of the proposed equipment maintenances services (i.e., telephone support, field service technician support hours, on-site response time, etc.) with the offer. Subline Item 0001AC, Consumable products. Offeror must provide a description or listing of all controls, calibrators, ancillary items and other consumable supplies that will be provided in the Cost-Per-Test (CPT) program. Line Items 0002 & 0003, Reagents. Offeror to provide the CPT rates for the minimum use quantities of the CPT program. Line Items 0003 & 0004, Reagents. Offeror to provide the CPT rates for tests performed over the minimum use quantities of the CPT program. (b). The requesting activity for this solicitation utilizes a separate Cepheid reagents contract for their testing requirements. In addition to the reagents that will be provided in the resultant contract to this solicitation, any proposed equal analyzer must be able to perform testing with the following Cepheid reagents: Xpert SA Nasal Complete, Item Number: GXSACOMP-10 Xpert CT/NG US-IVD, Item Number: GXCT/NG-120 Xpert Carba-R, Item Number: GXCARBAR-10 Xpert Xpress Flu, Item Number: XPRSFLU-10 Xpert MTB/RIF, Item Number: GXMTB/RIF-US-10 (c). This solicitation includes FAR Clause 52.228-5, Insurance Work on a Government Installation. The successful offeror must provide a copy of their insurance or provide a statement to the Contracting Officer indicating that they have enough insurance prior to contract performance. (d). This solicitation includes VAAR Clause 852.219-75, Subcontracting Commitments Monitoring and Compliance. The resultant contract for this solicitation is anticipated to be more than $700,000.00. The successful offeror must provide a subcontracting plan or a statement to the Contracting Officer describing why subcontracting possibilities are not available prior to contract performance. (e). This solicitation includes FAR Provision 52.209-7, Information Regarding Responsibility Matters. If the Offeror s online representations and certifications does not state the maintenance of the entities active registration in the System for Award Management (SAM) complied with the requirements to report proceedings data in accordance with FAR 52.509-7 and with the requirements to report executive compensation data in accordance with FAR 52.204-10, Reporting Executive Compensation and First-Tier Subcontract Awards, the Offeror must submit a completed copy of FAR Provision 52.209-7 with their offer. (f). This solicitation includes FAR Provision 52.237-2, Site Visit. A formal site visit will not be conducted during the solicitation. It is the responsibility of the Offeror to understand the site conditions prior to submitting an offer. (xiv) The Defense Priorities and Allocations System (DPAS) does not apply to this acquisition. (xv) Offers are due by 29 August, by 3:00 PM CST. Offers must be submitted by email (see size limit note below) to Vance Farrell at vance.farrell@va.gov. (xvi) Questions regarding information in the solicitation must be submitted in writing to the Point of Contact (POC), Vance Farrell, by email at vance.farrell@va.gov. Due to email constraints by the VA it is requested that emails being sent to the POC be no larger than 5MB. Multiple emails can be sent to the POC for an offer. If an offer is sent in multiple parts, it is recommended that a confirmation request be included in the final email, for the POC to acknowledge that all (parts) of the offer have been received.
