Pruett@VA.GOV Description TIERED EVALUATIONS INCLUDING SMALL BUSINESS CONCERNS: This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alternative, a tiered evaluation for VOSB concerns, or in the alternative, a set-aside for other small business concerns with HUBZone small business concerns and 8(a) participants having priority. If award cannot be made, the solicitation will be cancelled and the requirement resolicited. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This Request for Quotations (RFQ) number is 36C24419Q1064. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 effective January 24, 2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 541990, All Other Professional, Scientific, and Technical Services, with a small business size standard of $15 Million. The VA Medical Center of Wilmington, DE, is seeking a contractor who shall provide all resources necessary to accomplish the services and deliverables described in this statement of work (SOW), except as may otherwise be specified. The contractor shall provide a Health Physicist, Radiation Safety officer for Radiation protection and health physics survey services. Additionally, the contractor shall provide dosimetry badge dissemination, collection and data interpretation, routine inspection and performance testing of all radiological equipment, review and become a point person for radiology safety meetings. The Health Physicist must be able to respond and be on site within 1 hour of notification in the event of radiological events occurs. All interested companies shall provide quotations for the following: The anticipated Period of Performance base year plus (4) -12 month options. Contract award and includes the following Contract Line Item Numbers. Please see attachment 2. All work shall be completed in accordance with the Statement of Work (SOW). The SOW is included as Attachment 1 to this request for quote. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services. Since an award will be based on initial responses, offerors are highly encouraged to quote their most competitive pricing in their initial response. An award will be made to the offeror who is a Small Business which provides the lowest price and meets the requirements of the SOW. The Schedule of Supplies and Services is included in this request for quote. Please complete and return the Business Associate Agreement with your RFQ. Please provide GSA schedule Contract Number, Cage Code, NAICS Code, and Name of Point of Contact, Phone Number and Email address. Any award resulting from this Request for Quote will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k).
