- VA NEW YORK HARBOR HEALTHCARE SYSTEM RADIOLOGY SERVICE Manhattan Campus 423 E 23 Street New York, NY 10010 STATEMENT OF WORK DE-INSTALLATION AND REMOVAL OF X-RAY EQUIPMENTS Introduction and Background The VA NYHHS has an emergency requirement to un-install and remove of all old and obsolete X-Ray equipment located on 423 East 23rd St. New York, NY. This project requires an experienced registered contractor to design and execute the job. The requirement is an urgent and compelling need that needs to be completed by August 10 so new equipment can be installed to continue construction project for new Hybrid OR. Equipment are X-Ray machines that are required for veteran patient diagnosis Objective The Contractor shall provide all labor, material tools, equipment, supervision and other related items required to complete the project as per scope of work and specifications. All expenses towards mobilization at site and demobilization including bringing in equipment, workforce and materials, dismantling the equipment, clearing the site etc. shall be deemed to be included in the rates quoted by the contractor against various items of schedule of rates and no separate payment on such expenses shall be entertained. Summary of the Major Tasks Un-install/Removal of X-Ray Equipment from the following locations: ROOM Numbers: 1605, 1597, 6604D, 6594C, 6600B, 6694C, 6557, 6616, 6540B Dispose of all X-Ray equipment in accordance with Federal and State regulations Protect walls and floor finishes. Protect electrical connections 2.2 Specific Requirements 1. A site visit is required to verify existing conditions, specifications and measurements to get a full understanding of the work to be performed. Contractor shall be responsible for verification of existing conditions ensuring to ascertain the site conditions that may affect required equipment clearances, electrical, plumbing, medical gas and mechanical requirements. 2. Un-install and remove equipment, and accessories in accordance with the need of the service. 3. All work must be during regular hours as provided by the Engineering Service. Normal work hours are 7:00AM to 5:00PM Monday through Friday except for Federal Holidays. If the Contractor desires to work on Saturday, Sunday, holidays, or outside the project site s normal working hours the contractor must submit a written request for approval to the VA-COR at least seven (7) working days prior to the proposed start of such work. Due to emergency requirements for hybrid OR project, contractor will remove x ray equipment during the weekend. 4. Demo and remove all or any hardware accessories and similar items in place that are a part of the equipment s. 5. The contactor will be incompliance with the following NFPA 70 and the Master Construction Specifications Master Format 2004 (MF04), Division 25 6. It is the responsibility of the contractor to verify all quantities, field conditions and measurements prior to starting work. The contractor shall notify the COR of any differing site conditions prior to starting. 7. The Contractor shall provide an English-Speaking working supervisor on-site at all times when work is being performed and for the duration of this project. Supervisor must have a minimum of five (5) years of experience as a supervisor on five (5) projects of this complexity. Submission of references required as well as prior work experiences. Supervisor must be approved prior to any work being performed. No work shall begin without the working supervisor being approved. 8. The contractor shall dispose of the old X-ray equipment in accordance with safety regulations. 9. The Contractor use the freight/service elevators. And shall post visible construction signs and barriers prior to beginning any work. 10. The Contractor is responsible for any damage caused by his employees. All damages shall be repaired and/or replaced at no additional cost to the government. 11. Contractor shall submit schedule to VA for approval. The Contractor shall not perform any work on utility services without prior coordination with COR(Contract Officer Representative). The contractor shall not interfere in any way with Hospital and Pharmacy operations. 12. The Contractor shall complete all work within the specified number of calendar days. 13. The contractor shall provide construction signage at location of work. Signage shall comply with OSHA, Life Safety and NFPA requirements. 14. The contractor shall perform cleanup at any time when work area becomes cluttered and/or restricts traffic. Nevertheless, work area must be cleaned at the end of the each work shift. After completions, re-install all removed items. Exceptional care must be taken to assure tidiness of work. 15. Upon completion of the work all paint cans, other materials, containers, debris and protective coverings shall be cleaned up and removed from site and the Contractor shall leave the area in a clean, neat, and orderly condition satisfactory to the Contracting Officer. 16. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). 17. Work under this contract shall be carried out strictly in accordance with specifications attached and will meet US and Local codes. General Requirements The contractor shall be required to have VA issued identification badges for all of his/her personnel working on VA property. Anyone observed without VA identification will be asked to leave VA property. No VA identification, no work, no exception. There will be no smoking at this facility except in the smoking shelter, located in the rear of the facility. No exception. Those caught violating this requirement will be required to leave the facility and/or be escorted off the facility by Police. The contractor is responsible for obtaining his/her own vehicle parking. Vendor personnel must complete VA privacy and information security awareness training. This is a mandatory training for all VA and non-VA employees working at this facility. This training must be completed annually. Completion certification must be provided for each employee that will be onsite, if applicable. The contractor shall ensure that his/her personnel conduct themselves in a professional manner; i.e., no profanity; proper attire; no discourteous behavior to VA employees, etc. In the performance of all work, the workers must be incompliance with safety regulations governing worker safety (OSHA: 29 CFR 1926 Safety and Health Regulations for Construction; NFPA, Life Safety Codes, etc.). Place of Performance All work is to be performed in the address listed below NYHHCS New York Campus Radiology Service Location 423 East 23rd Street New York, NY 10010 Safety 4.1 Contractor is responsible and shall continue management and implementation of a safety and health program throughout construction. 4.2 The Contracting Officer and VA staff reserve the right to suspend work when patient, staff and contractor's safety and health program is considered to be inadequate or non-complying. 4.3 Contractor shall provide all Personal Protective Equipment for the workers as per the requirement of the site. Work will be stopped in case the proper protection equipment is not found with the workers and the lapse of time shall be at the Contractor s expense. 4.4 Contractor will not leave the work site in an unsafe condition or any other condition that might cause injury to personnel, damage to existing work, plants or equipment.
