POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company Name: Address: Point of Contact: Phone, Fax, and Email: DUNS number: Cage Code: Type of business, (e.g. Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a), HUB Zone, Women Owned Small Business, or Large Business): 2) Statement of Capability that demonstrates ability of providing equipment 3) Manufacturer: YES or NO 4) Distributor: YES or NO Will items/equipment be obtained from a SDVOSB/Small Business manufacturer? YES or NO Are the items/equipment on a FSS/GSA contract? YES or NO. If yes, please provide the contract number and list the items/equipment that are on the FSS/GSA contract. VAMC requires upgrading their two current OrthoScan C-Arm systems. Both units are operating on an outdated Operating System and require upgraded to ITEM# 915-0013-07Video Processing Boards, FD w/MDI, FD-OR -WIN7 (assembled) Minimum Technical Specifications: The upgrade of the C-Arms should meet the following minimum specifications: Upgrade two OrthoScan Units Unit #1: Serial Number: 5H0147 Unit #2: Serial Number: 5H0175 Both units are operating on software version 4.9.5.9 Both systems be upgraded to Windows 7 All hardware required to do so included All labor and travel included NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (08-AUG-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
